颅顶体表定位丘脑穿刺血肿引流术治疗老年丘脑出血破入脑室患者的效果评价

摘　　要：目的 探讨颅顶体表定位丘脑穿刺血肿引流术治疗老年丘脑出血破入脑室患者的临床效果。方法 82例老年丘脑出血破入脑室患者随机分为两组,对照组行双侧脑室钻孔引流术治疗,研究组行颅顶体表定位丘脑穿刺血肿引流术治疗,比较两组的手术指标、 NIHSS评分及术后并发症发生率。结果 研究组的术后24 h血肿清除率高于对照组,意识恢复时间、引流管留置时间、 ICU住院时间均短于对照组,术后15 d NIHSS评分低于对照组(P均<0.05)。研究组的术后并发症发生率为7.32%,低于对照组的26.83%(P<0.05)。结论 颅顶体表定位丘脑穿刺血肿引流术治疗老年丘脑出血破入脑室患者可获得显著效果。Objective To explore the clinical effect of thalamic puncture and hematoma drainage by cranio-parietal surface localization in the treatment of elderly patients with thalamic hemorrhage ruptured into the ventricle. Methods 82 elderly patients with thalamic hemorrhage ruptured into the ventricle were randomly divided into two groups. The control group was treated with bilateral ventricle drilling and drainage, and the study group was treated with thalamic puncture and hematoma drainage by cranio-parietal surface localization. The surgery indicators, NIHSS score and incidence of postoperative complications were compared between the two groups. Results The hematoma clearance rate at 24 h after surgery of the study group was higher than that of the control group, the consciousness recovery time,drainage tube indwelling time and ICU hospitalization time were shorter than those of the control group, and the NIHSS score at 15 d after surgery was significantly lower than that of the control group(all P <0.05). The incidence of postoperative complications of the study group was 7.32%, lower than 26.83% of the control group(P <0.05). Conclusions Thalamic puncture and hematoma drainage by cranio-parietal surface localization in the treatment of elderly patients with thalamic hemorrhage ruptured into the ventricle can achieve significant effect.
