摘  要:目的观察术前预适应训练对老年骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fractures,OVCF)患者PKP手术的影响。方法选取60例拟行PKP的老年OVCF患者,按照随机数字表法分为观察组(术前预适应训练,n=30)、对照组(术前无干预,n=30),比较两组患者术中生命体征,手术前后椎体高度、Oswestry功能障碍指数(Oswestry disability index,ODI),术后VAS评分和活动适应度、术中依从性、首次下床活动时间及并发症。结果观察组术中心率、平均动脉压、术后24 h ODI评分和术后12、24 h的VAS评分显著低于对照组(P<0.05),血氧饱和度显著高于对照组(P<0.05);观察组患者术后活动适应度显著高于对照组(P<0.05);观察组患者术中依从性显著优于对照组,首次下床活动时间显著短于对照组(P<0.05);两组术后24 h的椎体高度、并发症发生率差异无统计学意义(P>0.05)。结论术前预适应训练有利于改善老年OVCF患者PKP术中的生命体征,减轻术后疼痛,提高术后活动适应度,促进功能恢复。Objective To observe the effect of preoperative preadaptation training on elderly patients with OVCF treated with PKP.Methods Sixty elderly patients with OVCF who planned to undergo PKP were selected,and randomly divided into observation group(preoperative preadaptation training,n=30)and control group(no intervention,n=30).The two groups were compared in terms of intraoperative vital signs,vertebral height,ODI score,VAS score,activity fitness,intraoperative compliance,first ambulation time,and complications.Results The intraoperative HR and MAP,24 h postoperative ODI score,12 h and 24 h postoperative VAS scores of the observation group were lower than those of the control group(P<0.05),and SpO_(2)was higher than that of the control group(P<0.05).The observation group had better activity fitness than the control group after operation(P<0.05).The intraoperative compliance of observation group was better than that of control group,and the first ambulation time was shorter than that of control group(P<0.05).The 24 h postoperative vertebral height and the incidence of complications showed no significant difference between the groups(P>0.05).Conclusion Preoperative preadaptation training is beneficial to improve the intraoperative vital signs,alleviate postoperative pain,improve and postoperative activity fitness,and promote functional recovery.
