摘 要:目的 探讨ASK模型联合持续质量改进法在医疗护理员手卫生培训中的效果。方法 采用前后对比研究方法,选取本院55名医疗护理员为研究对象,以ASK模型为理论框架进行为期1w的手卫生培训后,采用持续质量改进法进行为期3个月的手卫生培训效果持续监测。比较培训前后医疗护理员手卫生理论和操作考核成绩、五个手卫生指征(接触患者前、清洁操作前、接触排泄物后、接触患者后、接触患者周围环境后)的依从率及正确率。结果 55名医疗护理员均完成研究。经过3个阶段的ASK模型联合持续质量改进培训后,医疗护理员3个质控周期的理论考核成绩较培训前明显提高,差异有统计学意义(均P<0.001);3个质控周期的操作考核成绩较培训前明显提高,各个时间段比较,差异有统计学意义(均P<0.01)。五个手卫生指征的总依从率和总正确率分别从培训前15.87%、38.24%逐步提高至58.87%、66.41%,各个时间段比较,差异具有统计学意义(均P<0.001)。结论 ASK模型联合持续质量改进法能有效提高医疗护理员的手卫生理论知识、操作技能、手卫生的依从率和正确率,为完善医疗护理员培训和管理机制提供了一种新方法。Objective To explore the effect of ASK model combined with continuous quality improvement in hand hygiene training for nursing assistants. Methods With the before-and-after comparative study, a total of 55 nursing assistants in our hospital were selected as the study subjects. After one week of hand hygiene training with the ASK model as the theoretical framework, the effect of hand hygiene training was continuously monitored for 3 months using the method of continuous quality improvement. The effects before and after the training were compared in terms of the examination on hand hygiene theory and practical operation, the compliance rate and the correct rate of the five hand hygiene indicators which were before touching the patients,before cleaning the patients,after touching the excrement,after touching the patients and after touching the surroundings of the patients.Results A total of 55 nursing assistants completed the study.After three stages of the ASK model combined with Continuous Quality Improvement Training,the results of examination in hand hygiene theory were significantly improved by comparison with the pre-training (P<0.001).The operation performances were significantly improved in all the three cycles of training (P<0.001).The total compliance rate and the total correct rate of the five hand hygiene indicators increased from 15.86%and 38.24%to 58.87%and 66.41%,respectively,with statistically significant difference as compared with the pretraining (P<0.001).Conclusions ASK model combined with continuous quality improvement in hand hygiene training for nursing assistants can effectively improve the theoretical knowledge,operational skills and the rates of compliance and correctness of hand hygiene.It provides a reference for improving the quality in training and management of nursing assistants.
