健胰祛湿降糖方治疗肥胖型2型糖尿病脾虚湿困证临床研究

摘　　要：目的:观察健胰祛湿降糖方治疗肥胖型2型糖尿病脾虚湿困证的临床疗效。方法:选取2020年1月至2021年1月在上饶市广丰区中医院内分泌科进行治疗的肥胖型2型糖尿病脾虚湿困证患者100例,随机分为对照组和治疗组,每组各50例。对照组采用饮食和运动疗法加二甲双胍肠溶片治疗,治疗组在对照组治疗的基础上采用健胰祛湿降糖方治疗,4周为1个疗程,共治疗3个疗程。比较两组患者治疗前后血糖水平、胃肠激素水平、胰岛素抵抗与分泌情况、中医证候积分等。结果:两组患者治疗后血糖水平均降低,且治疗组低于对照组,差异有统计学意义(P<0.05)。两组患者治疗后胃肠激素含量均升高,且治疗组高于对照组,差异有统计学意义(P<0.05)。两组患者治疗后胰岛素抵抗指数降低,胰岛素分泌指数升高,且组间比较,差异有统计学意义(P<0.05)。两组患者治疗后中医证候积分均降低,且治疗组低于对照组,差异有统计学意义(P<0.05)。结论:健胰祛湿降糖方治疗肥胖型2型糖尿病脾虚湿困证,可以改善患者症状,降低血糖,提高胃肠激素含量。Objective:To observe the clinical efficacy of Pancreas-Strengthening and Dampness-Removing and Sugur-Lowering Decoction on obese type Ⅱ diabetes with spleen deficiency and dampness encumbering syndrome.Methods:A total of 100 patients with obese type Ⅱ diabetes with spleen deficiency and dampness encumbering syndrome treated in the Department of Endocrinology,Guangfeng District Hospital of Traditional Chinese Medicine of Shangrao from January 2020 to January 2021 were selected.According to the random number table method,they were divided into the control group and the treatment group,with 50 cases in each group.The control group was treated with diet and exercise therapy plus Metformin Enteric Coated Tablets,while the treatment group was treated with Pancreas-Strengthening and Dampness-Removing and Sugur-Lowering Decoction on the basis of the treatment of the control group.Four weeks was a course of treatment,and each group received a total of three courses of treatment.The blood glucose level,gastrointestinal hormone,insulin resistance and secretion,TCM syndrome score and so on were compared between the two groups before and after treatment.Results:The blood sugar level in the two groups decreased on average after treatment,and the blood sugar level of the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).The content of gastrointestinal hormones in the two groups increased after treatment,and the index of the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the insulin resistance index decreased,while the insulin secretion index increased,and there were significant differences between the two groups(P<0.05).The TCM syndrome scores of the two groups decreased after treatment,and the index of the treatment group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Pancreas-Strengthening and Dampness-Removing and
