摘　　要：目的:观察针刺结合康复训练治疗脑卒中后痉挛性足内翻的临床疗效。方法:选取2019年1月至2020年12月上海市长宁区天山中医医院收治的脑卒中后痉挛性足内翻患者120例,按照随机数字表法分为观察组与对照组,每组60例。对照组给予常规康复训练,观察组在对照组治疗的基础上给予针刺治疗。观察两组患者痉挛程度、临床疗效及治疗前后步行能力、生活能力变化情况。结果:观察组优良率为91.67%,对照组优良率为81.67%,观察组优良率高于对照组,差异有统计学意义(P<0.05);观察组痉挛程度优于对照组,差异有统计学意义(P<0.05);两组治疗后美国国立卫生研究院卒中量表(national institute of health stroke scale,NIHSS)评分低于治疗前,且观察组NIHSS评分低于对照组,差异有统计学意义(P<0.05);两组治疗后Holden步行功能分级、日常生活活动能力量表评分均高于治疗前,且观察组Holden步行功能分级、日常生活活动能力量表评分高于对照组,差异有统计学意义(P<0.05)。结论:脑卒中痉挛性足内翻患者给予针刺结合康复训练治疗能够降低痉挛程度,提升患者步行能力、生活能力。Objective:To observe the clinical efficacy of acupuncture combined with rehabilitation training on spastic foot varus after stroke.Methods:A total of 120 patients with spastic foot varus after stroke treated in Shanghai Changning Tianshan Traditional Chinese Medicine Hospital from January 2019 to December 2020 were randomly divided into the observation group and the control group,with 60 cases in each group.The control group was given routine rehabilitation training,while the observation group was given acupuncture treatment on the basis of the control group.The degree of spasticity,clinical efficacy and the changes of walking ability and living ability before and after treatment were observed.Results:The excellent rate of the observation group was 91.67%,which was significantly higher than that of the control group:81.67%,and the difference was statistically significant(P<0.05).The degree of spasticity in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The national institute of health stroke scale(NIHSS) score of the two groups after treatment was lower than that before treatment,and the NIHSS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of Holden walking function scale and activity of daily living scale in the two groups were higher than those before treatment,and the above indexes in the observation group were higher than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Acupuncture combined with rehabilitation training can reduce the degree of spasticity in stroke patients with spastic foot varus,and improve their walking ability and living ability.
