针刀疗法联合中药熏蒸治疗髌股关节骨关节炎30例

摘　　要：目的:观察针刀疗法联合中药熏蒸治疗髌股关节骨关节炎的临床疗效。方法:选取2020年9月至2021年3月黑龙江中医药大学第一附属医院骨伤一科门诊治疗的髌股关节骨关节炎患者60例,按照随机数字表法分为治疗组与对照组,每组30例。治疗组给予针刀疗法联合中药熏蒸治疗,对照组给予针灸疗法联合中药熏蒸治疗。观察两组患者临床疗效、复发情况及治疗前后疼痛视觉模拟评分(visual analogue score,VAS)、Lysholm膝关节功能评分(Lysholm knee function score,LKS)变化情况。结果:与治疗前比较,两组治疗后VAS评分均降低,LKS评分均提高,差异具有统计学意义(P<0.05),治疗组VAS评分低于对照组、LKS评分高于对照组,但差异无统计学意义(P>0.05);治疗组有效率为96.7%,对照组有效率为63.3%,治疗组有效率高于对照组,差异有统计学意(P<0.05);7个月后随访,治疗组复发率为6.7%,对照组复发率为33.3%,治疗组复发率低于对照组,差异有统计学意义(P<0.05)。结论:针刀疗法联合中药熏蒸治疗髌股关节骨关节炎能减轻患者疼痛,改善患者膝关节功能,降低髌股关节骨关节炎复发率。Objective:To observe the clinical efficacy of needle-knife therapy combined with Chinese medicinal fumigation on patella-femoral osteoarthritis(PFOA).Methods:A total of 60 patients with PFOA treated in No.1 Outpatient Department of Orthopedics and Traumatology,First Affiliated Hospital,Heilongjiang University of Chinese Medicine from September 2020 to March 2021 were randomly divided into the treatment group and the control group,with 30 cases in each group.The treatment group was treated with needle-knife therapy combined with Chinese medicinal fumigation,and the control group was treated with acupuncture combined with Chinese medicinal fumigation.The clinical efficacy,recurrence and changes of visual analog score(VAS) and Lysholm knee function score(LKS) before and after treatment were observed.Results:Compared with those before treatment,the VAS score of the two groups decreased and the LKS score increased after treatment,and all the differences were statistically significant(P<0.05).The VAS score of the treatment group was lower than that of the control group,and the LKS score was higher than that of the control group,but all the differences were not statistically significant(P>0.05).The effective rate of the treatment group was 96.7% and that of the control group was 63.3%.The effective rate of the treatment group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After 7 months of follow-up,the recurrence rate in the treatment group was 6.7% and that in the control group was 33.3%.The recurrence rate in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Needle-knife therapy combined with Chinese medicinal fumigation in the treatment of PFOA can reduce pain,improve knee function and reduce the recurrence rate of patients.
