摘　　要：目的了解Barthel指数≥90分的自然分娩产妇角色行为强化的内在因素,为促进产妇顺利实现角色转换提供参考依据。方法采取立意抽样方法抽取某院2019年9—10月自然分娩的产妇为研究对象,采用质性研究方法对12例Barthel指数≥90分的自然分娩产妇进行半结构式、面对面的深度访谈,运用Colaizzi资料分析原则进行资料整理分析,提炼影响因素主题。结果共访谈12名自然分娩的初产妇,年龄(30.50±3.07)岁;共提取自然分娩产妇角色行为强化八个副主题(包括双方父母的过度保护、配偶的过度保护、新生儿护理知识缺乏、产褥期护理知识缺乏、产后出血护理知识缺乏、缺乏自信、焦虑心理、失败的经验)、三个主题(即照顾者过度保护、相关知识缺乏、自我效能感低)。结论自然分娩产妇角色行为强化由主、客观多方面因素所致,护理人员可根据自然分娩产妇角色行为强化的影响因素,通过健康教育及共同参与式管理方式,改善其知识结构,鼓励产妇主动进行自我护理,促进自然分娩产妇角色行为的早期转变,顺利完成被动照顾替代过渡到自我护理。Objective To understand the internal factors of role behavior reinforcement for women in natural childbirth with Barthel index≥90,so as to provide reference basis for the smooth realization of role transition for women in childbirth.Methods Women who gave birth naturally in September and October of 2019 in a hospital were selected as research objects.Qualitative research method was used to conduct semi-structured,face-to-face in-depth interviews with 12 natural parturient with Barthel index≥90.Colaizzi data analysis principle was used to for data collation and analysis,and topics extraction.Results A total of 12 primiparas who aged 30.50±3.07 years,were included.Total extract natural childbirth puerpera role behavior strengthen eight subtopic(including both parents'overprotection,spouses of excessive protection,lack of knowledge of neonatal care,nursing knowledge in puerperium lack,lack of postpartum hemorrhage nursing knowledge,lack of self-confidence,anxiety and experience of failure),and three themes(caregivers overprotection,lack of relevant knowledge,and low self-efficacy).Conclusion Accor-ding to the factors of the role behavior in natural parturient,health education and participatory management can be used to change the traditional concept,encourage the puerpera to take the initiative in self-care,promote the early change of role behavior,and successfully complete the transition from passive-care to self-care.
