血府逐瘀汤加减治疗原发性稳定型微血管心绞痛患者的临床研究

摘　　要：目的 探讨血府逐瘀汤加减治疗原发性稳定型微血管心绞痛患者的临床疗效及对超敏C反应蛋白(hs-CRP)、白细胞介素-1β(IL-1β)及血管内皮因子的影响。方法 将60例患者随机分为中药组与对照组各30例,对照组给予常规药物,中药组在对照组基础上加血府逐瘀汤加减,两组均用药4周。观察两组运动平板Duke评分、心绞痛症状评分及疗效、心电图ST段下移幅度及疗效、中医症候评分及疗效、西雅图心绞痛量表(SAQ)评价,血清炎症指标、血管内皮功能因子等。结果 与对照组比较,治疗后中药组患者心绞痛症状评分、中医证候评分均降低,Duke评分、SAQ评分及其疗效均升高(P <0.05),hs-CRP、IL-1β、内皮素(ET-1)表达均降低,一氧化氮(NO)表达升高(P <0.05),两组ST段下移幅度及疗效差异无统计学意义(P> 0.05)。结论 血府逐瘀汤加减联合常规治疗能够显著改善患者心绞痛症状、中医证候,下调炎症因子表达,调节血管内皮功能,提高生活质量。Objective:To explore the clinical effect of Xuefu Zhuyu Decoction on patients with primary stable microvascular angina and its observation on IL-1β and vascular endothelial function.Methods:A total of 60 cases of patients were randomly divided into traditional Chinese medicine(TCM)group and control group,namely 30cases in each group.The control group was treated with conventional drugs,and the TCM group was treated with Xuefu Zhuyu Decoction on the basis of the control group.Both groups were treated for 4 weeks.Duke exercise treadmill score,angina symptom score,electrocardiogram(ECG)ST segment depression amplitude and curative effect,TCM symptom score and curative effect,Seattle Angina Questionnaire(SAQ),as well as serum inflammatory factors,vascular endothelial function factors,platelet related factors were observed.Results:Compared with the control group,the symptom score of angina pectoris and TCM in the TCM group were lower,while Duke,SAQ score and curative effect were higher(P < 0.05);the levels of IL-1β,ET-1 were lower,and NO were higher(P <0.05);there was no significant difference in ST segment depression amplitude and curative effect of ECG between the two groups after treatment(P > 0.05).Conclusion:Xuefu Zhuyu Decoction combined with routine treatment can significantly improve the symptoms of angina pectoris and TCM symptoms,reduce inflammatory factors,regulate vascular endothelial function,and improve the quality of life.
