摘　　要：目的:探讨腹腔双套管匀速冲洗引流在胰腺部分切除与胰腺外伤手术后胰瘘治疗中的作用。方法:选取2019年1月-2021年6月单县中心医院肝胆外科收治的70例行胰腺切除与胰腺外伤手术后胰瘘患者进行研究,随机分为两组,各35例。研究组进行腹腔双套管匀速冲洗引流,对照组进行常规冲洗引流。比较两组患者治疗后的并发症发生率、腹腔冲洗时间、住院时间、胰周引流量、腹腔引流液中淀粉酶水平。结果:研究组并发症发生率低于对照组,差异具有统计学意义(P<0.05);研究组腹腔冲洗时间、住院时间短于对照组,差异具有统计学意义(P<0.05);研究组胰周引流量、腹腔引流液中淀粉酶水平均低于对照组,差异具有统计学意义(P<0.05)。结论:腹腔双套管匀速冲洗引流对胰腺部分切除与胰腺外伤手术后胰瘘的治疗效果较好,可明显降低并发症发生率,缩短患者腹腔冲洗时间和住院时间。Objective:To explore the role of double-cannula peritoneal irrigation and drainage at a constant speed in the treatment of pancreatic fistula after partial pancreatectomy and pancreatic trauma surgery.Methods:A total of 70 patients with pancreatic fistula after pancreatectomy and pancreatic trauma surgery who were admitted to Shanxian Central Hospital from January 2019 to June 2021 were selected for the study,and they were randomly divided into two groups,with 35 cases in each group.The study group underwent double-cannula peritoneal irrigation and drainage at a constant speed,and the control group underwent routine irrigation and drainage.The complication rate,peritoneal irrigation time,length of hospital stay,peripancreatic drainage volume,and amylase level in abdominal drainage liquid were compared between the two groups after treatment.Results:The incidence of complications in the study group was lower than that in the control group,with a statistically significant difference(P<0.05).The peritoneal irrigation time and length of hospital stay in the study group were shorter than those in the control group,with a statistically significant difference(P<0.05).The peripancreatic drainage volume and the level of amylase in the peritoneal drainage fluid in the study group were lower than those in the control group,with a statistically significant difference(P<0.05).Conclusion:Double-cannula peritoneal irrigation and drainage at a constant speed has a better effect in the treatment of pancreatic fistula after partial pancreatectomy and pancreatic trauma surgery,which can significantly reduce the incidence of complications,shorten the patient’s peritoneal irrigation time and hospital stay.
