彩色多普勒超声心动图对老年性退行性心瓣膜病变的诊断价值

摘　　要：目的:探讨彩色多普勒超声心动图对老年性退行性心瓣膜病变的诊断特点及临床意义。方法:选取2018年1月-2021年1月在湖北省荣军医院收治的1 600例老年性心脏病变患者作为研究对象,按照各病例检查手段的不同分为两组,各800例。对照组患者采取二维超声心动图进行诊断,而观察组患者采取彩色多普勒超声心动图进行诊断,对比两组患者老年性退行性心瓣膜病变的确诊率、误诊率以及患者确诊前后的负性情绪评分。结果:对照组患者退行性心瓣膜病变确诊率高于观察组,差异具有统计学意义(P<0.05);观察组器质性心脏病误诊率低于对照组,差异具有统计学意义(P<0.05);确诊后,对照组焦虑自评量表、抑郁自评量表评分均高于观察组,差异具有统计学意义(P<0.05)。结论:对于老年性退行性心瓣膜病变,彩色多普勒超声心动图的诊断精准率要优于二维超声心动图,应该在老年性心脏病变患者的临床治疗中进行推广使用。Objective:To investigate the diagnostic characteristics and clinical significance of color Doppler echocardiography on senile degenerative heart valve disease.Methods:From January 2018 to January 2021,1600 senile patients with heart disease in Hubei Veterans Hospital were selected as the study subjects.According to different examination methods,the patients were divided into two groups with 800 cases in each group.The control group was diagnosed by two-dimensional echocardiography and the observation group was diagnosed by color Doppler echocardiography.The conformed diagnosis rate and misdiagnosis rate of senile degenerative heart valve disease,as well as negative mood scores of patients before and after confirmed diagnosis were compared in two groups of patients.Results:The confirmed diagnosis rate of degenerative heart valve disease in the control group was higher than that in the observation group,and the difference was statistically significant(P<0.05);the misdiagnosis rate of organic heart disease in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05);after diagnosis,the scores of anxiety self-rating scale and depression self-rating scale in the control group were higher than those in the observation group,and the difference was statistically significant(P<0.05).Conclusion:For the diagnosis of senile degenerative heart valve disease,the diagnostic accuracy of color Doppler echocardiography is better than that of two-dimensional echocardiography,which should be popularized in the clinical treatment of senile patients with heart disease.
