抗苗勒氏管激素与卵巢型子宫内膜异位症严重程度及治疗后性激素水平的关系探讨

摘　　要：目的:探讨抗苗勒氏管激素(Anti-mullerian hormone,AMH)与卵巢型子宫内膜异位症严重程度及其治疗后性激素的关系。方法:选取2019年3月—2021年3月本院收治的卵巢型子宫内膜异位症患者60例作为研究对象,Ⅲ期、Ⅳ期各30例,分别设为研究组和对照组。均予以腹腔镜联合促性腺激素释放激素激动剂(Gonadotropin-releasing hormone agonist,GnRH-a)治疗,比较两组AMH水平和性激素水平,并分析AMH与性激素的相关性。结果:研究组术后AMH、E2、FSH、LH、PRL、T水平低于对照组,差异有统计学意义(P<0.05);两组术后1年妊娠率比较,差异无统计学意义(P>0.05);Pearson相关性分析显示,AMH表达水平与E2、FSH、LH、PRL、T均呈正相关(r=0.651、0.671、0.735、0.658、0.721,P<0.05)。结论:卵巢型子宫内膜异位症患者AMH水平与严重程度及性激素水平密切相关,临床在予以腹腔镜联合药物治疗后可采用AMH评估患者卵巢功能恢复情况,有助于判断患者预后。Objective: To investigate the relationship between anti-Millerian Hormone(AMH) and the severity of ovarian endometriosis and sex hormones after treatment. Methods: A total of 60 patients with ovarian endometriosis admitted to our hospital from March 2019 to March 2021 were selected as the research objects, with 30 cases in stage Ⅲ and 30 cases in stage Ⅳ, respectively, and set as the research group and the control group. All patients were treated with laparoscopy combined with Gonadotropin-releasing hormone agonist(GnRH-a). The levels of AMH and sex hormone were compared between the two groups, and the correlation between AMH and sex hormone was analyzed. Results: The levels of AMH,E2, FSH, LH, PRL and T in the study group were lower than those in the control group, and the difference was statistically significant(P<0.05). There was no significant difference in pregnancy rate between the two groups at 1 year after operation(P>0.05). Pearson correlation analysis showed that the expression level of AMH was positively correlated with E2, FSH,LH, PRL and T(r=0.651, 0.671, 0.735, 0.658, 0.721, P<0.05). Conclusion: The level of AMH in patients with ovarian endometriosis is closely related to the severity and sex hormone levels. Clinically, AMH can be used to evaluate the recovery of ovarian function in patients after laparoscopic combined with drug treatment, which is helpful to determine the prognosis of patients.
