玻璃体注药联合石斛明目丸口服对增殖型糖尿病视网膜病变患者手术治疗效果及视力的影响研究

摘　　要：目的探讨玻璃体注药联合石斛明目丸口服对增殖型糖尿病视网膜病变患者手术治疗效果及视力的影响。方法选择2018年4月至2019年4月进行治疗增殖型糖尿病视网膜病变患者86例(单眼患病)作为对象,随机数字表法分为对照组(43例)和观察组(43例)。对照组予以玻璃体注药切除术,观察组予以玻璃体注药联合石斛明目丸口服治疗。比较治疗后2组患者的临床症状改善情况,血糖水平、视力水平及并发症发生率。结果治疗后观察组眼压、视网膜静脉循环时间、视野缺损均低于对照组(P<0.05),RNFL厚度较对照组高(P<0.05);观察组治疗后视力恢复水平高于对照组患者(P<0.05);观察组患者在治疗后出现恶心呕吐、眼睛干涩、玻璃体再次出血及医源性裂孔的总发生率为7.0%,低于对照组37.2%(P>0.05)。结论玻璃体注药联合石斛明目丸口服对增殖型糖尿病视网膜病变患者手术治疗效果显著,有效改善患者的的临床症状,提高视力水平,且并发症较少,值得推广应用。Objective To investigate the effects of vitreous injection combined with Shihu Mingmu Wan oral administration on surgical treatment and visual acuity in patients with proliferative diabetic retinopathy.Methods From April 2018 to April 2019,86 patients with proliferative diabetic retinopathy(monocular disease)were selected as subjects and divided into control group(n=43)and observation group(n=43)by random number table method.The control group was treated with vitrectomy and the observation group was treated with vitrectomy combined with oral dendrobium.The improvement of clinical symptoms,blood glucose level,visual acuity level and incidence of complications were compared between the two groups after treatment.Results After treatment,intraocular pressure,retinal meridians circulation time and visual field defect in the observation group were lower than those in the control group(P<0.05),and retinal nerve fibre laye thickness was higher than that in the control group(P<0.05).The visual acuity recovery of observation group was higher than that of control group(P<0.05).The total incidence of nausea and vomiting,dry eyes,vitreous rebleeding and iatrogenic hiatus in observation group was 7.0%,which was lower than 37.2%in control group(P>0.05).Conclusion Vitreous injection combined with Shihu Mingmu Wan oral administration has significant effect on the surgical treatment of patients with proliferative diabetic retinopathy,and can effectively improve the clinical symptoms of patients,improve the level of vision,and reduce complications,which is worthy of popularization and application.
