中药溻渍方对全膝关节置换术后功能恢复的影响

摘　　要：目的观察中药溻渍方对全膝关节置换术(TKA)后功能恢复的影响。方法选取2020年12月至2021年7月中国中医科学院望京医院关节一科初次行TKA患者90例作为研究对象,术后2周拆线后采用随机数字表法分为对照组和试验组,每组各45例。对照组给于扶他林,试验组给于中药溻渍方,干预2周后比较两组视觉模拟评分法(VAS)评分、下肢周径差值、股四头肌Kendall百分比、关节活动度(ROM)及美国特种外科医院(HSS)膝关节评分。结果干预后,两组VAS评分低于干预前,且试验组低于对照组,两组HSS评分高于干预前,且试验组高于对照组(P<0.05)。干预后,两组下肢肿胀程度低于干预前,且试验组低于对照组,两组主动股四头肌肌力及ROM高于干预前,且试验组高于对照组(P<0.05)。给药过程中,两组均未出现不良反应。结论中药溻渍方能够显著缓解TKA患者的肢体疼痛、肿胀,改善关节功能,促进TKA术后康复。Objective To observe the effect of Chinese herb Tazi Decoction on functional recovery after total knee arthroplasty(TKA).Methods From December 2020 to July 2021,90 patients in Wangjing Hospital of China Academy of Chinese Medical Sciences who were undergoing TKA were selected as the study subjects.Two weeks after stitches removal,the patients were divided into control group and experimental group by random number table method,with 45 cases in each group.The control group was given Voltaren,and the experimental group was given Chinese herb Tazi Decoction.After two weeks of intervention,visual analogue scale(VAS)score,lower limb circumference difference value,Kendall percentage of quadriceps femoris muscle,range of motion(ROM),and knee score of American Hospital for Special Surgery(HSS)were compared between the two groups.Results After intervention,VAS score of two groups were lower than those before intervention,and that in the experimental group was lower than that in the control group,HSS score of two groups were higher than those before intervention,and that in the experimental group was higher than that in the control group(P<0.05).After intervention,two groups of lower limb swelling degree were lower than those before intervention,and that in the experimental group was lower than that in the control group.Both groups of active quadriceps muscle and ROM were higher than those before intervention,and those in the experimental group were higher than those in the control group(P<0.05).There were no adverse reactions in experimental group and control group during administration.Conclusion Chinese herb Tazi Decoction can significantly relieve the pain and swelling of limbs of patients after TKA,improve joint function,and promote the rehabilitation of TKA.
