摘　　要：目的观察鲜红斑痣的皮肤镜特征,为临床诊治提供参考。方法选择2019年1月1日—2019年7月13日期我科门诊接诊的100例鲜红斑痣患儿,均于VEOS HD2皮肤镜下观察红斑痣特征。观察不同年龄分层、发病类型患者镜下特征差异。结果鲜红斑痣临床表现为边缘清楚、不规则红斑,按压褪色或不完全褪色。皮肤镜下检查结果示:21例(21.00%)为点状血管分布模式,23例(23.00%)网状血管分布模式,29例(29.00%)点状模式+网状模式,18例(18.00%)不规则分支血管模式,9例(9.00%)均一红色区域模式。不同年龄鲜红斑痣镜下特征差异有统计学意义(P<0.05),其中7~12岁组以均一红色区域模式(35.00%)和不规则分支血管模式(35.00%)为主,2个月~3岁组以点状血管分布模式(23.08%)、网状血管分布模式(43.59%)为主。不同发病类型鲜红斑痣镜下特征差异有统计学意义(P<0.05),多发型以点状模式+网状模式(33.33%)和不规则分支血管模式(30.56%)为主,单发型以点状血管分布模式(21.88%)、网状血管分布模式(28.13%)、点状模式+网状模式(26.56%)居多。结论鲜红斑痣有典型的皮肤镜下特征,皮肤镜可为鲜红斑痣诊断和临床治疗提供一定参考信息。Objective In order to investigate the dermoscopic characteristics of port wine stains and provide reference for clinical diagnosis and treatment.Methods A total of 100 children with port wine stains who were admitted to our outpatient clinic from January 1,2019 to July 13,2019 were enrolled.The characteristics of erythema were observed under(VEOS HD2)dermoscope.The differences in microscopic features of patients with different age groups and disease types were observed.Results The clinical manifestations of port wine stains were clear edges,irregular erythema,fading or incomplete fading.The results of dermatoscopy showed that 21 cases(21.00%)were punctate vascular distribution patterns,23 cases(23.00%)were reticular vascular distribution patterns,29 cases(29.00%)were punctate patterns plus and reticular patterns,and 18 cases(18.00%)irregular branch vessel pattern,9 cases(9.00%)were in a red zone mode.The characteristics of the different ages of the erythema were significantly different(P<0.05).Among them,the 7 to 12 years old group was dominated by the uniform red regional pattern(35.00%)and the irregular branch vessel pattern(35.00%);and the 2 months to 3 years old group was mainly dominated by the punctate vascular distribution pattern(23.08%)and the reticular vascular distribution pattern(43.59%).The characteristics of different incidence types of port wine stains were also statistically different(P<0.05).The multiple hair styles were mainly punctate pattern+reticular pattern(33.33%)and irregular branch vascular pattern(30.56%).The punctate vascular distribution pattern(21.88%),the reticular vascular distribution pattern(28.13%),and the punctate pattern+reticular pattern(26.56%)were in the majority.Conclusion Port wine stains have typical dermoscopic features.Dermatoscopy can provide some reference information for the diagnosis and clinical treatment of port wine stains.
