加味皱肺丸辅助治疗老年慢性阻塞性肺疾病稳定期患者62例

摘　　要：目的 探讨加味皱肺丸辅助治疗老年慢性阻塞性肺疾病(COPD)稳定期患者的临床效果。方法 选取老年COPD稳定期患者124例,按照随机数字表法分为治疗组及对照组各62例。对照组采用常规治疗方法,治疗组在对照组基础上加用加味皱肺丸。治疗12周后观察两组患者的临床疗效、中医症状评分、肺功能、改良版英国医学研究委员会呼吸问卷评分(mMRC)、COPD患者自我评估测试问卷评分(CAT)、6 min步行试验及血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、IL-8水平。结果 治疗组总有效率95.2%高于对照组83.9%(P<0.05),治疗后治疗组和对照组中医症状评分、mMRC与CAT评分均较前下降;6 min步行试验、每分钟最大通气量占预测值百分比(MVV%pred)、第1秒用力呼气量占预测值百分比(FEV_(1)%pred)、FEV_(1)/FVC均较治疗前升高,且治疗组优于对照组,差异有统计学意义(P<0.05);治疗组和对照组患者血清TNF-α、IL-6、IL-8治疗后均较治疗前下降,且治疗组低于对照组,差异有统计学意义(P<0.05)。结论 加味皱肺丸联合西医常规治疗可以改善老年COPD稳定期患者的临床症状、肺功能及运动耐量;减轻气道炎症反应,提高患者的治疗效果,且用药安全。Objective To investigate the clinical efficacy of Jiawei Zhoufei pills in the adjuvant treatment of elderly patients with stable chronic obstructive pulmonary disease(COPD). Methods A total of 124 elderly patients with stable COPD were selected and divided into treatment group and control group, with 62 patients in each group, according to the random number table method.The control group received conventional treatment, while the treatment group was additionally treated with Jiawei Zhoufei pills based on the control group.After the treatment for 12 weeks, the clinical efficacy, Traditional Chinese Medicine(TCM) symptom score, lung function, modified British Medical Research Council respiratory questionnaire(mMRC) score, COPD self-assessment test(CAT) score, 6-min walk test, and serum TNF-α,IL-6 and IL-8 levels of the two groups were observed. Results The total effective rate of the treatment group was 95.2%,which was higher than 83.9% of the control group(P<0.05).After the treatment, TCM symptom score, mMRC score, and CAT score were all decreased, while the 6-min walk test, MVV% pred, FEV_(1)% pred, and FEV_(1)/FVC were all increased in both the treatment group and the control group, compared with those before the treatment.The changes were more dramatic in the treatment group than those in the control group, and the differences were statistically significant(P<0.05).Serum TNF-α,IL-6 and IL-8 in both groups after the treatment were lower than before, which were dramatic in the treatment group compared with the control group, the differences were statistically significant(P<0.05). Conclusion The combination of Jiawei Zhoufei pills with conventional western medicine treatment can improve the clinical symptoms, lung function, and exercise tolerance in elderly patients with stable COPD;It can reduce airway inflammation and improve the treatment effect of patients with high safety.
