摘 要:目的评价自行研制的充气压迫式膝关节伸直康复训练装置的临床应用效果。方法纳入自2019-02-2021-02完成的79例全膝关节置换术,术后手术侧膝关节均存在不同程度的屈曲挛缩,40例采用充气压迫式膝关节伸直康复训练装置进行膝关节伸直训练(观察组),39例由康复医师徒手进行膝关节伸直训练(对照组)。比较两组首次膝关节伸直训练时膝关节疼痛VAS评分、康复训练后膝关节屈曲挛缩改善程度、康复医师操作时间,以及术后1周膝关节HSS评分。结果79例术后能按要求完成膝关节伸直康复训练,训练过程中切口均未破损,未影响假体稳定性。观察组首次膝关节伸直训练时膝关节疼痛VAS评分低于对照组,康复医师操作时间明显短于对照组,差异有统计学意义(P<0.05)。观察组与对照组康复训练后膝关节屈曲挛缩改善程度、术后1周膝关节HSS评分差异无统计学意义(P>0.05)。结论全膝关节置换术后残留膝关节屈曲挛缩患者采用充气压迫式膝关节伸直康复训练装置进行膝关节伸直训练具有操作简单、压力均衡、适合长时间训练的优点,可替代康复医师完成大部分康复训练操作。Objective To evaluate the clinical application effect of self-developed inflatable compression knee extension rehabilitation training device for knee joint extension rehabilitation.Methods A total of 79 cases of total knee arthroplasty from February 2019 to February 2021 were included, and there were different degrees of flexion contracture in the knee joint after operation. Forty cases of knee joint extension training were performed with inflatable compression knee extension rehabilitation training device(observation group) while 39 patients underwent knee extension training by rehabilitation physicians(control group). The VAS score of knee joint pain during the first knee straightening training, the degree of improvement of knee joint flexion contracture after rehabilitation training, the operation time of the rehabilitation physician, and the knee joint HSS score 1week after the operation were compared between the two groups.Results Seventy-nine cases could complete the knee extension rehabilitation training as required after operation, and the incision was not damaged during the training, and the stability of the prosthesis was not affected. The VAS score of knee joint pain in the observation group during the first knee extension training was lower than that in the control group, and the operation time of the rehabilitation physician was significantly shorter than that in the control group, with statistical significance(P<0.05). There was no significant difference in the improvement of knee flexion contracture and the knee joint HSS score 1 week after rehabilitation training between the observation group and the control group(P>0.05).Conclusion For patients with residual knee flexion contracture after total knee arthroplasty, the inflatable compression knee extension rehabilitation training device is used for the advantages of simple operation, balanced pressure, and suitable for long-term training. It can replace the rehabilitation physician to complete most of the rehabilitation training operations.
