摘  要:目的 明确藏族男性中年慢性高原病(chronic mountain sickness, CMS)患者骨髓巨核细胞增殖、凋亡变化及相关蛋白表达情况。方法 通过分析藏族男性中年CMS患者外周血血小板计数及相关指标变化,进一步研究藏族男性中年CMS患者与藏族健康人群之间骨髓巨核细胞增殖、凋亡变化及相关蛋白表达情况的差异。选取藏族男性中年CMS患者(CMS组)及藏族健康体检者(CON组)各8例,检测血常规并留取骨髓液,用流式细胞仪检测骨髓巨核细胞G0G1、S、G2M,评估增殖率;应用Annexin V-FITC/PI双染法检测骨髓巨核细胞的凋亡率,并检测骨髓巨核细胞C-MPL、Bax、Bcl-2、caspase-3等蛋白的表达。结果 (1)CMS组PLT、PCT较CON组下降(P<0.001,P=0.004),P-LCR较CON组升高(P=0.009),而MPV、PDW两组间接近临界值(P=0.054,P=0.072);(2)流式细胞术检测发现,CMS组骨髓巨核细胞增殖率较CON组升高(P<0.001),凋亡率下降(P<0.001),但两组间骨髓巨核细胞TPO配体C-MPL和凋亡相关蛋白Bax、Bcl-2、caspase-3等表达水平均无显著性差异(均P>0.05)。结论 在高原环境下,本研究组藏族男性中年CMS患者PLT较藏族健康人群低;同时,巨核细胞凋亡减少,可能是血小板减少的原因之一。另外,骨髓巨核细胞增殖旺盛,可能是对血小板减少的一种代偿性反应。与汉族相关情形无差异。Objective To investigate the proliferation and apoptosis of megakaryocytes and the expression of related proteins in middle-aged Tibetan male patients with chronic mountain sickness(CMS).Methods The changes of proliferation and apoptosis of megakaryocyte as well as the expression of related proteins between middle-aged Tibetan male patients with CMS and Tibetan healthy people were studied by analyzing the variations of platelet count of peripheral blood and related parameters in middle-aged Tibetan male patients with CMS.Eight middle-aged Tibetan male patients with CMS(CMS group)and eight Tibetan healthy people(CON group)were selected to detect blood routine and take bone marrow fluid.Megakaryocytes G0 G1,S and G2 M were detected by flow cytometry to evaluate the proliferation rate;Annexin V-FITC/PI double staining was used to detect the apoptosis rate of megakaryocytes and the expression of C-MPL,Bax, Bcl-2,caspase-3 and other proteins in megakaryocytes.Results(1) The values of PLT and PCT in CMS group were lower than those in CON group(P<0.001,P=0.004),while the value of P-LCR was higher than that in CON group(P=0.009).The differences of MPV and PDW were close to crtical values between these two groups(P=0.054,P=0.072).(2)Flow cytometry showed that the proliferation rate of megakaryocytes in CMS group was higher than that in CON group(P<0.001)and the apoptosis rate was decreased(P<0.001).The expression levels of megakaryocytes TPO ligand C-MPL and apoptosis-related proteins Bax, Bcl-2 and caspase-3 between the two groups were not significantly different(all P>0.05).Conclusion The PLT of middle-aged Tibetan male patients with CMS in this study group is lower than that of Tibetan healthy people in the plateau environment;The decrease of megakaryocyte apoptosis may be one of the causes of thrombocytopenia.The vigorous proliferation of megakaryocytes may be a compensatory response to thrombocytopenia, which is not different from that of Han nationality.
