甲泼尼松龙冲击联合厄贝沙坦片治疗原发性系膜增生性肾小球肾炎的临床效果

摘　　要：目的探讨甲泼尼松龙冲击联合厄贝沙坦片治疗原发性系膜增生性肾小球肾炎(MsPGN)的临床效果。方法选取2017年1月至2019年6月我院收治的经明确病理诊断的MsPGN患者120例,随机分为观察组与对照组,每组各60例。对照组给予甲泼尼松龙治疗,观察组给予甲泼尼松龙冲击联合厄贝沙坦治疗,疗程均为4周,对比两组治疗前后的尿白蛋白、24 h尿蛋白定量、尿素氮、内生肌酐清除率水平,比较两组临床疗效及不良反应发生率。结果两组治疗后尿白蛋白、24 h尿蛋白定量均明显下降,且观察组优于对照组,组间比较差异均有统计学意义(t=18.649、7.400,均P<0.05)。观察组的缓解率高于对照组,组间比较差异有统计学意义(χ^(2)=5.885,P<0.05)。两组不良反应发生率比较,差异无统计学意义(χ^(2)=0.370,P>0.05)。结论甲泼尼松龙冲击联合厄贝沙坦片治疗MsPGN的疗效显著,不良反应轻,值得应用。Objective To investigate the clinical effect of methylprednisolone combined with irbesartan tablets on primary mesangial proliferative glomerulonephritis(MsPGN).Methods A total of 120 pathologically diagnosed MsPGN patients admitted in our hospital from January 2017 to June2019 were selected,and randomly divided into observation group and control group with 60 cases in each group.The patients in the control group were given methylprednisolone and in the observation group were given methylprednisolone combined with irbesartan tablets for four weeks.The urinary albumin,24-hour urinary protein,urinary nitrogen,endogenous creatinine clearance rate of the two groups before and after treatment were compared,and clinical efficacy and incidence of adverse reactions were compared between the two groups.Results After treatment,the urinary albumin and 24-hour urinary protein of the two groups were significantly decreased,and which in the observation group were better than that of the control group,the difference between groups were statistically significant(t=18.649、7.400,all P<0.05).The remission rate of the observation group was higher than the control group(χ^(2)=5.885,P<0.05).The incidence of adverse reactions of the two groups were compared,and the difference was no statistically significant(χ^(2)=0.370,P>0.05).Conclusion Methylprednisolone combined with irbesartan tablets for MsPGN has significant efficacy and mild adverse reactions,which is worthy of application.
