食管癌术后胸内吻合口瘘的诊治进展

摘　　要：胸内吻合口瘘是食管癌术后最常见且最严重的并发症之一,发生率3%~25%。吻合口瘘可导致患者胸腔感染、肺炎,需长期禁食,延长了住院时间,严重者需再次手术治疗或出现死亡等。由于吻合口瘘的症状、程度等不同,大多数常予保守治疗,内窥镜下治疗有支架、吻合夹、真空辅助闭合治疗及内镜下缝合等,必要时再次行手术缝合。本文就国内外对食管癌术后胸内吻合口瘘的诊断及治疗进展作一综述。Intrathoracic anastomotic leakage is one of the most common and serious complications after esophagectomy,with an incidence ranging from 3%to 25%.It can lead to thoracic infection and pneumonia,which require long-term fasting and prolonged hospitalization.In severe cases,reoperation is required,and even death occurs.Due to the different symptoms and degrees of anastomotic leakage,most patients are often treated conservatively,Endoscopic treatment includes stent,over-the-scope-clip,vacuum-assisted closure therapy and over-stitch,etc.Surgical suture may be performed again if necessary.This paper reviews the progress in the diagnosis and treatment of intrathracic anastomotic leakage after esophagectomy at home and abroad.
