多排螺旋CT在评价食管癌术后复发/转移中的价值

摘　　要：目的探讨多排螺旋CT在评价食管癌术后复发/转移中的价值。方法回顾性分析48例经病理证实术后复发/转移的食管癌患者螺旋CT影像资料,记录食管癌肿瘤复发/转移的类型、部位及时间,对相应数据进行分析。结果本研究48例食管癌患者中,肿瘤复发18例,其中食管胃吻合口部10例,食管残端部2例,残胃部6例;淋巴结转移35例,以3区(13例)和8区(16例)多见,两区以上淋巴结转移18例,腹膜后腹主动脉旁淋巴结转移8例,锁骨上窝淋巴结转移6例;肺部转移6例,肝脏转移7例,胰腺转移2例;48例复发/转移病例中,伴胸腔积液8例,双侧积液2例,单侧积液6例(均为手术侧)。肿瘤复发/转移多见于术后3~12个月内(P<0.05)。结论多排螺旋CT是评价食管癌术后复发/转移的有效手段。Objective To explore the value of multi-slice spiral CT in the evaluation of postoperative recurrence/metastasis of esophageal cancer.Methods The spiral CT imaging data of 48 patients with postoperative recurrence/metastasis of esophageal cancer confirmed by pathology were analyzed retrospectively.The types,sites and recurrence time of tumor recurrence/metastasis were recorded and statistically analyzed.Results Among 48 patients with recurrence/metastasis,digestive tract tumor recurred in 18 cases,10 including esophagogastric anastomosis,2 esophageal stump and 6 gastric stump;lymph node metastasis was found in 35 cases,mostly in 3R zone(13 cases)and 8 zone(16 cases).Lymph node metastasis in more than two zones was found in 18 cases,retroperitoneal paraaortic lymph nodes metastasis in 8 cases,and supraclavicular fossa lymph node metastasis in 6 cases.Lung metastasis in 6 cases,liver metastasis in 7 cases,pancreas metastasis in 2 cases.Pleural effusion was found in 8 cases,bilateral in 2 cases,and unilateral in 6 cases(all on the operative side).Recurrence/metastasis was more common from 3 months to 12 months after operation(P<0.05).Conclusion Multi slice spiral CT is an effective way to evaluate the postoperative recurrence/metastasis of esophageal cancer.
