摘  要:目的分析血浆微小RNA-378(miR-378)、热休克转录因子1(HSF1)与慢性心力衰竭(CHF)病人左心室重构的相关性。方法选取2017年5月—2019年5月于唐山市开滦总医院心内科住院治疗的CHF病人137例作为CHF组,依据美国纽约心脏病协会(NYHA)心功能分级标准对CHF病人进行分级,其中Ⅰ级或Ⅱ级45例,Ⅲ级50例,Ⅳ级42例;另选取同期因急性心肌梗死于本院住院治疗的60例病人作为对照组。所有病人均接受超声心动图检测;实时荧光定量聚合酶链式反应(qRT-PCR)法检测受试者血浆miRNA-378水平,酶联免疫吸附法(ELISA)检测受试者血浆HSF1水平;Pearson法分析血浆miR-378、HSF1水平与左心室重构指标左心室重量指数(LVMI)、左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)相关性;多元线性逐步回归模型分析影响CHF病人LVMI水平的相关因素。结果与对照组比较,CHF组心功能Ⅰ级或Ⅱ级、Ⅲ级、Ⅳ级组病人血浆LVMI、LVEDV水平依次升高(P<0.05),miR-378、HSF1、LVEF水平依次降低(P<0.05)。Pearson法分析显示,CHF组病人血浆miR-378、HSF1与LVMI、LVEDV均呈负相关(P<0.005),与LVEF呈正相关(P<0.05)。多元线性逐步回归分析显示,血浆miR-378、HSF1是LVMI的影响因素(P<0.05)。结论血浆miR-378、HSF1水平降低可能与CHF病人左心室重构的发生发展有关,可为临床诊断CHF病人发生左心室重构及评估病情严重程度提供依据。Objective To analyze the correlation between plasma microRNA-378(miR-378),heat shock transcription factor 1(HSF1)and left ventricular remodeling in patients with chronic heart failure(CHF).Methods A total of 137 patients with CHF were selected as the CHF group.According to the New York Heart Association(NYHA)cardiac function classification standard,there were 45 patients with CHF gradeⅠorⅡ,50 patients with CHF gradeⅢ,and 42 patients with CHF gradeⅣ.Another 60 patients with acute myocardial infarction during the same period were selected as the control group.Echocardiography was performed in all patients.The plasma miR-378 level was detected by real-time fluorescence quantitative PCR(QRT-PCR)and the plasma HSF1 level was detected by enzyme-linked immunosorbent assay(ELISA).The correlation between plasma miR-378 and HSF1 levels and left ventricular mass index(LVMI),left ventricular ejection fraction(LVEF),and left ventricular end-diastolic volume(LVEDV)were analyzed by Pearson method.Multiple linear stepwise regression model was used to analyze the factors influencing LVMI level in CHF patients.Results Compared with the control group,the levels of LVMI and LVEDV in CHF group,with cardiac function gradeⅠorⅡ,gradeⅢand gradeⅣwere successively increased(P<0.05),and the levels of miR-378,HSF1,and LVEF were successively decreased(P<0.05).Pearson analysis showed that plasma miR-378 and HSF1 were negatively correlated with LVMI and LVEDV in CHF group(P<0.05),and positively correlated with LVEF(P<0.05).Multiple linear stepwise regression analysis showed that the influence of plasma miR-378 and HSF1 on LVMI was statistically significant(P<0.05).Conclusion The decrease of miR-378 and HSF1 in plasma may be related to the occurrence and development of left ventricular remodeling in patients with CHF,which can provide a basis for clinical diagnosis of left ventricular remodeling in patients with CHF and evaluation of the severity of the disease.
