经双极等离子前列腺剜除术治疗良性前列腺增生的疗效及对尿道功能、性功能的影响

摘　　要：目的探讨良性前列腺增生患者经双极等离子前列腺剜除术治疗的疗效及对其尿道功能、性功能的影响。方法选取2020年1月至2021年9月卢氏县中医院收治的82例患者随机分为常规组41例,观察组41例。常规组给予单极电切镜治疗,观察组通过经双极等离子前列腺剜除术治疗。对比治疗前后两组临床疗效,尿道功能水平,性功能及并发症发生率。结果两组苏醒时间差异无统计学意义(P>0.05),治疗后两组治疗时间差异有统计学意义(P<0.05),观察组治疗时间短于常规组,差异有统计学意义(P<0.05)。治疗前,两组最大尿流量、残余尿量差异无统计学意义(P>0.05),治疗后两组患者最大尿流量均升高、残余尿量均降低,差异有统计学意义(P<0.05),且观察组最大尿流量更高,残余尿量更低,差异有统计学意义(P<0.05)。观察组前列腺症状评分、勃起功能障碍低于常规组,差异有统计学意义(P<0.05)。治疗期间观察组有1例膀胱痉挛,1例尿滞留,并发症发生率为4.88%(2/41),常规组有1例尿滞留,并发症发生率为2.44%(1/41),两组并发症发生率比较,差异无统计学意义(校正χ^(2)=0.000,P=1.000)。结论良性前列腺增生患者经双极等离子前列腺剜除术治疗能够缩短治疗时间,同时能够改善尿道功能水平,提高性功能。Objective To investigate the efficacy of bipolar plasma enucleation of prostate in patients with benign prostatic hyperplasia(BPH)and its effect on urethral function and sexual function.Methods A total of 82 patients admitted to our hospital from January 2020 to September 2021 were randomly divided into routine group(41 cases)and observation group(41 cases).The routine group was treated with unipolar electrocision,and the observation group was treated with bipolar plasma enucleation of prostate.The clinical efficacy,urethral function,sexual function and complication rate were compared between the two groups before and after treatment.Results There was no significant difference in the recovery time between the two groups(P>0.05),but there was significant difference in the treatment time between the two groups after treatment(P<0.05),the treatment time of the observation group was shorter than that of the routine group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the maximum urine flow and residual urine volume between the two groups(P>0.05).After treatment,the maximum urine flow increased and the residual urine volume decreased in both groups(P<0.05),and the maximum urine flow was higher and the residual urine volume was lower in the observation group(P<0.05).The prostate symptom score and erectile dysfunction in the observation group were lower than those in the routine group(P<0.05).During the treatment period,there was 1 case of bladder spasm and 1 case of urinary retention in the observation group,the incidence of complications was 4.88%(2/41),and 1 case of urinary retention in the routine group,the incidence of complications was 2.44%(1/41),there was no significant difference between the two groups(correctedχ^(2)=0.000,P=1.000).Conclusion Bipolar plasma enucleation can shorten the treatment time,improve the level of urethral function and sexual function in patients with benign prostatic hyperplasia.
