中药外治法联合外科手术治疗胫骨创伤后骨髓炎骨质皮肤缺损的价值

摘　　要：目的探析中药外治法联合外科手术治疗胫骨创伤后骨髓炎骨质皮肤缺损的价值。方法将安阳市人民医院2018年6月至2019年6月期间收治的76例胫骨创伤后骨髓炎骨质皮肤缺损患者纳为研究对象,按随机数字表法将其分为研究组与对照组,每组38例。对照组给予外科手术治疗,研究组在此基础上联合中药外治,对比两组疗效,治疗前与治疗3、5周后的血沉水平,感染率,复发率,创面愈合时间,骨质愈合时间,视觉模拟疼痛(VAS)评分。结果研究组治疗有效率高于对照组,差异有统计学意义(P<0.05);两组治疗前的血沉水平,差异无统计学意义(P>0.05),研究组治疗后的第3、5周血沉水平均低于对照组;研究组感染率、复发率低于对照组,差异有统计学意义(P<0.05);研究组创面愈合平均时间、骨质愈合平均时间分别为(20.13±5.47)d和(5.69±1.33)个月,明显短于对照组的(29.83±5.87)d和(7.36±1.68)个月,差异有统计学意义(P<0.05);研究组VAS评分为(1.72±0.93)分,明显低于对照组的(3.07±1.25)分,差异有统计学意义(P<0.05)。结论中药外治法联合外科手术治疗胫骨创伤后骨髓炎骨质皮肤缺损的疗效显著,能降低感染的发生率与疾病的复发率,并能加快患者切口、创面、骨质的愈合,减轻患者痛苦,改善其预后。Objective To explore the value of external treatment of Traditional Chinese Medicine combined with surgery in the treatment of bone and skin defects in post-traumatic osteomyelitis of the tibia.Methods A total of 76 patients with posttraumatic osteomyelitis bone and skin defects of the tibia admitted to our hospital from June 2018 to June 2019 were enrolled as the research objects.They were divided into the study group and the control group according to the random number table method,with 38 cases in each group.The control group was given surgical treatment,and the study group was combined with external treatment of Traditional Chinese Medicine on this basis,and the efficacy,erythrocyte sedimentation rate,infection rate,recurrence rate,wound healing time,bone healing time and visual analog pain(VAS)score before treatment and after 3 and 5 weeks of treatment were compared between the two groups.Results The effective rate of treatment in the study group was higher than that in the control group(P<0.05);There was no significant difference in the ESR levels between the two groups before treatment(P>0.05),and the ESR levels in the study group at the 3rd and 5th week after treatment were lower than those in the control group;The infection rate and recurrence rate in the study group were lower than those in the control group(P<0.05);The average time for wound healing and bone healing in the study group were(20.13±5.47)days and(5.69±1.33)months,respectively,which were significantly shorter than those in the control group.(29.83±5.87)days and(7.36±1.68)months in the group(P<0.05);The VAS score in the study group was(1.72±0.93),which was significantly lower than that in the control group(3.07±1.25)(P<0.05).Conclusion External treatment of Traditional Chinese Medicine combined with surgery has a significant effect on the treatment of bone and skin defects in posttraumatic osteomyelitis of the tibia,which can reduce the incidence of infection and the recurrence rate of the disease,and can accelerate the healing of inci
