透明帽辅助无痛胃镜下食管异物取出术的疗效评价

摘　　要：目的评价透明帽辅助无痛胃镜下食管异物取出术的临床疗效。方法抽取登封市人民医院2018年5月至2021年5月接收的80例消化道异物患者,将采用常规无痛胃镜下食管异物取出术治疗的40例患者纳入对照组,采用透明帽辅助无痛胃镜下食管异物取出术治疗的40例患者纳入观察组。比较两组患者术前(T0)、置入胃镜时(T1)、取异物时(T2)的心率与平均动脉压(MAP)变化;比较两组患者内镜下视野清晰度、内镜下操作时间与手术成功情况,比较两组并发症发生率。结果T1、T2时,两组心率与MAP均较T0时升高,且T2时的心率与MAP均较T1时升高,但观察组各时点心率与MAP均低于对照组(P均<0.05)。观察组视野清晰度总优良率、手术成功率分别为95.00%(38/40)、97.50%(39/40),均高于对照组的80.00%(32/40)、80.00%(32/40),P均<0.05。观察组内镜下操作时间[(9.46±2.27)min]少于对照组[(11.63±2.38)min],P<0.05。两组术后并发症发生率比较差异未见统计学意义(P>0.05)。结论透明帽辅助无痛胃镜下食管异物取出术可提高消化道异物患者内镜下视野清晰度与手术成功率,缩短内镜下操作时间,利于稳定患者血流动力学。Objective To investigate the effect of cap-assisted painless gastroscopic removal of esophagus foreign bodies.Methods Eighty patients with gastrointestinal foreign bodies treated in Dengfeng People’s Hospital from May 2018 to May 2021 were selected.Forty patients treated by routine painless gastroscopic removal of esophagus foreign bodies were included in the control group,and 40 patients treated by cap-assisted painless gastroscopic removal of esophagus foreign bodies were included in the observation group.The changes of heart rate and mean arterial pressure(MAP)before operation(T0),when gastroscope was inserted(T1)and when foreign body was removed(T2)were compared between the two groups;the visual field definition,endoscopic operation time and operation success were compared between the two groups,and the incidences of complications were compared between the two groups.Results The heart rate and MAP at T1 and T2 in both groups were higher than those at T0,and the heart rate and MAP at T2 were higher than those at T1,but the heart rate and MAP at each time in the observation group were lower than those in the control group(all P<0.05).The total excellent and good rate of visual field definition and the success rate of operation in the observation group were 95.00%(38/40)and 97.50%(39/40),respectively,which were higher than 80.00%(32/40)and 80.00%(32/40)in the control group(all P<0.05).The endoscopic operation time in the observation group was(9.46±2.27)min,which was lower than the(11.63±2.38)min in the control group(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions The cap-assisted painless gastroscopic removal of esophagus foreign bodies can improve the clarity of endoscopic visual field and the success rate of surgery,shorten the operation time,and stabilize the hemodynamics of patients with gastrointestinal foreign bodies.
