趾腓侧皮瓣修复手指指腹皮肤软组织缺损的疗效分析

摘　　要：目的 分析游离趾腓侧皮瓣修复手指指腹皮肤软组织缺损创面的方法和临床效果。方法 2014年12月-2019年12月,将36例手指指腹皮肤缺损患者作为研究对象,随机分为两组,观察组18例,采用游离趾腓侧皮瓣进行移植修复,对照组18例,采用指动脉逆行岛状皮瓣进行移植修复,观察比较两种术式术后并发症发生情况及手指指腹感觉功能恢复情况。结果 36例皮瓣全部成活,术后并发症,观察组1例(5.56%),对照组8例(44.44%),观察组术后并发症发生率少于对照组,差异具有统计学意义(P<0.05);术后6个月两点辨别觉,观察组4~6 mm,对照组6~8 mm,差异有统计学意义(P<0.05),观察组优于对照组。结论 游离趾腓侧皮瓣修复手指指腹缺损,具有术后并发症少、恢复快、供区损伤小的特点。皮瓣质地好,感觉恢复好,是修复指腹皮肤软组织缺损的良好方案。Objective To explore the clinical application effects of repairing finger pulp defect using free great toe fibular lateral flap. Methods From Dec. 2014 to Dec. 2019, 36 patients who suffered from finger pulp defect were taken as the research objects,and these patients were randomy divided into two groups. The observation group with 18 cases was treated by free great toe fibular lateral flap. The control group with 18cases was treated by reverse digital artery island flap. Results All 36 flaps survived. Postoperative complications rate in the observation group was 1 case(5.56%), while in the control group was 8 cases(44.44%). The differences between two groups was significant(P <0.05). 6 months after surgery, The observation group was 6 ~8 mm better than the control group which is 6 ~8 mm, and the differences had significance(P <0.05). Conclusion Repairing finger pulp defect using free great toe fibular lateral flap, leads to little postoperative complications, faster postoperative recovery and little damage to the donor area. The flap has good texture and feels well. Great toe fibular feels is an excellent solution for repairing the defect of finger pulp.
