摘  要:目的探讨糖尿病肾病(DN)肾小球系膜细胞葡萄糖转运蛋白1(GLUT1)的表达情况与系膜细胞衰老之间的关系。方法根据高糖处理浓度不同将人肾小球系膜细胞(HRMCs)分为NG组(5.6 mmol/L葡萄糖)和HG组(30 mmol/L葡萄糖),应用Western blot技术检测两组肾小球系膜细胞GLUT1、衰老因子P16、P27表达情况。应用酶联免疫吸附法(ELISA)检测并比较两组研究对象(DN组和对照组)的血清GLUT1、P16、P27表达情况。比较两组研究对象的空腹血糖、尿微量白蛋白、24 h尿蛋白定量、尿素氮、血肌酐水平,并将其与GLUT1、P16、P27进行Pearson相关性分析。结果与对照组比较,DN组的空腹血糖、尿微量白蛋白、24 h尿蛋白定量、尿素氮、血肌酐水平均明显升高(P<0.05)。Western blot检测结果显示,与NG组比较,HG组的GLUT1、P16、P27蛋白在不同时间点的表达量均明显增强(P<0.05)。与对照组比较,DN组的血清GLUT1、P16、P27水平均增高(P<0.05)。Pearson相关性分析结果显示,DN患者血清GLUT1水平与空腹血糖、尿微量白蛋白、24 h尿蛋白定量、尿素氮、血肌酐呈显著正相关,P16水平与尿素氮、血肌酐呈显著正相关,P27水平与24 h尿蛋白定量、尿素氮、血肌酐呈显著正相关(P<0.05)。结论高糖环境下肾小球系膜细胞与DN患者GLUT1、P16、P27的表达呈高度一致,且三者的高表达均与肾功能下降明显相关,提示GLUT1表达很可能通过P16、P27高表达这一衰老机制参与DN肾功能损害的病理过程。Objective To investigate the relationship between expression of glucose transporter 1(GLUT1) of glomerular mesangial cells in diabetic nephropathy(DN) and the senescence of mesangial cells. Methods Human renal mesangial cells(HRMCs) were divided into NG group(5.6 mmol/L glucose) and HG group(30 mmol/L glucose) according to the concentration of high glucose treatment, the expression of GLUT1 of glomerular mesangial cells, and senescence factors P16 and P27 were detected by Western blot. The expression of serum GLUT1, P16 and P27 in the research objects of two groups(DN group and control group) were detected by enzyme-linked immunosorbent assay(ELISA) and compared.Fasting blood glucose, urinary microalbumin, 24 h urinary protein, urea nitrogen and serum creatinine levels were compared between the two groups, and the Pearson correlation analysis was performed between them and GLUT1, P16 and P27. Results Compared with the control group, the levels of fasting blood glucose, urinary microalbumin, 24 h urinary protein, urea nitrogen and serum creatinine in the DN group significantly increased(P<0.05). The results of Western blot showed that compared with the NG group, the expression of GLUT1, P16 and P27 proteins in the HG group significantly increased at different time points(P<0.05). Compared with the control group, the levels of serum GLUT1, P16 and P27 in the DN group increased(P <0.05). Pearson correlation analysis results showed that the level of serum GLUT1 was positively correlated with fasting blood glucose, urinary microalbumin, 24 h urinary protein, urea nitrogen and serum creatinine, the level of P16 was positively correlated with urea nitrogen and serum creatinine, and the level of P27 was positively correlated with 24 h urinary protein, urea nitrogen and serum creatinine(P<0.05). Conclusion The expression of GLUT1, P16 and P27 in glomerular mesangial cells under high glucose environment and DN patients is highly consistent,and the high expression of the three is significantly related to the decline of rena
