通腑活血汤对腹腔镜下子宫全切术后患者康复的影响

摘　　要：目的:观察通腑活血汤对腹腔镜下子宫全切术后患者康复的影响。方法:将60例腹腔镜下子宫全切术后患者随机分为试验组和对照组各30例。对照组术后给予常规治疗,试验组在对照组治疗基础上口服通腑活血汤。比较两组患者术后不同时间腹痛程度,术后首次下床活动时间、首次排气时间和首次排便时间,白细胞、C反应蛋白(C-reaction protein,CRP)水平及腹胀临床疗效。结果:试验组患者术后第1天、第2天、第3天疼痛程度低于对照组,差异均有统计学意义(P<0.05);试验组治疗后中重度腹胀发生率为10.00%,对照组发生率为36.67%,两组比较,差异有统计学意义(P<0.05);试验组患者首次排气时间和首次排便时间均低于对照组,差异均有统计学意义(P<0.05);术后第3天,试验组患者白细胞、CRP水平均低于对照组,差异均有统计学意义(P<0.05)。结论:通腑活血汤用于腹腔镜下子宫全切术后有助于提高镇痛疗效,可缩短胃肠功能恢复所需时间,减轻腹胀程度及炎症反应,加快患者康复。Objective:To observe the effect of Tongfu Huoxue Decoction on the rehabilitation of patients after laparoscopic total hysterectomy.Methods:60 patients after laparoscopic total hysterectomy were randomly divided into the experimental group and the control group,with 30 cases in each group.The control group was given routine treatment after the operation,and the experimental group was given Tongfu Huoxue decoction orally on the basis of the treatment of the control group.The degree of abdominal pain at different times after operation,the time of first getting out of bed,the time of first farting and the first defecation,the levels of white blood cells and C-reaction protein(CRP) and the clinical efficacy of abdominal distension were compared between the two groups of patients.Results:The pain degree of patients in the experimental group was lower than that in the control group on the 1 st,2 nd,and 3 rd days after the operation,and the differences were statistically significant(P<0.05).After treatment,the incidence of moderate to severe abdominal distension in the experimental group was 10.00%,and the incidence in the control group was 36.67%,and the difference was statistically significant(P<0.05).The time to first fart and the time to the first defecation in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).On the 3 rd day after the operation,the levels of white blood cells and CRP in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Tongfu Huoxue Decoction after laparoscopic total hysterectomy can help to improve the analgesic effect,shorten the time required for the recovery of gastrointestinal function,reduce the degree of abdominal distension,and inflammatory reaction,and speed up the recovery of patients.
