温中除痹汤加减联合针刺治疗脾肾两虚型老年原发性骨质疏松症临床研究

摘　　要：目的:观察温中除痹汤加减联合针刺治疗脾肾两虚型老年原发性骨质疏松症(POP)的临床疗效。方法:选取60例脾肾两虚型老年POP患者,根据挂号顺序单双号分为对照组与试验组各30例。2组均给予常规治疗,对照组在此基础上给予针刺治疗,试验组在对照组基础上给予温中除痹汤加减治疗。2组均治疗3个月。比较2组临床疗效及治疗前后腰椎与左侧股骨颈骨密度(BMD)、血清骨形成标志物[骨钙素(BGP)、碱性磷酸酶(ALP)]水平。结果:经秩和检验,试验组临床疗效优于对照组(P<0.05)。试验组总有效率96.67%,高于对照组73.33%(P<0.05)。治疗后,2组腰椎、左侧股骨颈BMD均较治疗前升高(P<0.05),试验组腰椎、左侧股骨颈BMD均高于对照组(P<0.05)。治疗后,2组BGP、ALP水平均较治疗前降低(P<0.05),试验组BGP、ALP水平均低于对照组(P<0.05)。结论:温中除痹汤加减联合针刺治疗脾肾两虚型老年POP疗效较好,可提高患者的BMD,改善血清骨形成标志物水平。Objective:To observe the clinical effect of modified Wenzhong Chubi tang combined with acupuncture for senile patients with primary osteoporosis(POP)of spleen-stomach weakness type.Methods:A total of 60 cases of senile patients with POP of spleen-stomach weakness type were selected and divided into the control group and the experiment group according to the odd-even number of registration order,with 30 cases in each group.Both groups were given routine treatment,the control group was additionally treated with acupuncture,and the experiment group was additionally treated with modified Wenzhong Chubi tang based on the treatment of the control group.Both groups were treated for three months.The bone mineral density(BMD)of lumbar vertebra and left femoral neck and levels of serum bone formation markers including bone gla protein(BGP)and alkaline phosphatase(ALP)before and after treatment as well as the clinical effects in the two groups were compared.Results:The clinical effect in the experiment group was better than that in the control group(P<0.05).The total effective rate was 96.67%in the experiment group,higher than that of 73.33%in the control group(P<0.05).After treatment,BMD of lumbar vertebra and left femoral neck in the two groups were increased when compared with those before treatment(P<0.05),and the BMD of lumbar vertebra and left femoral neck in the experiment group were higher than those in the control group(P<0.05).After treatment,the levels of BGP and ALP in the two groups were decreased when compared with those before treatment(P<0.05),and the two levels above in the experiment group were lower than those in the control group(P<0.05).Conclusion:The therapy of modified Wenzhong Chubi tang combined with acupuncture has a good curative effect in treating senile patients with POP of spleen-stomach weakness type,which can improve their BMD and the levels of serum bone formation markers.
