基于温痹散应用火龙灸治疗风寒湿痹型膝骨关节炎临床研究

摘　　要：目的:观察基于温痹散应用火龙灸治疗风寒湿痹型膝骨关节炎(KOA)的临床效果。方法:选择风寒湿痹型KOA患者74例,按随机数字表法分为对照组和观察组各37例。对照组给予塞来西布胶囊等常规治疗,观察组在对照组的基础上以温痹散为药饼进行火龙灸治疗。2组均以7 d为1个疗程,共治疗3个疗程。观察2组治疗前后疼痛视觉模拟评分法(VAS)评分、骨关节炎指数量表(WOMAC)评分和血清炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平,比较2组治疗安全性。结果:观察组总有效率为91.89%,对照组为72.97%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组VAS、WOMAC评分均较治疗前降低(P<0.05),且研究组上述2项评分均低于对照组(P<0.05)。治疗后,2组血清TNF-α、IL-6水平均较治疗前明显降低(P<0.05),且研究组血清TNF-α、IL-6水平均低于对照组(P<0.05)。治疗期间,2组均未出现明显药物不良反应,研究组亦未出现皮肤烫伤和过敏症状,安全性良好。结论:基于温痹散应用龙灸治疗风寒湿痹型KOA患者,可显著缓解患者疼痛,有效减轻机体炎症反应,改善关节功能。Objective:To observe the clinical effect of fire-dragon moxibustion with Wenbi powder on knee osteoarthritis(KOA)of wind-cold and damp-impediment type.Methods:A total of 74 KOA patients of wind-cold and damp-impediment type were randomly divided into the control group and the observation group,with 37 cases in each group.The control group was given cailaixibu capsules and other conventional treatment,and the observation group was additionally given fire-dragon moxibustion with Wenbi powder based on the treatment of the control group.Both groups were treated for three courses,with seven days being one course.The scores of Visual Analogue Scale(VAS),Western Ontario and McMaster Universities Arthritis Index(WOMAC)and levels of inflammatory factors in serum including tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)before and after treatment were observed,and the treatment safety was compared between the two groups.Results:The total effective rate was 91.89%in the observation group and 72.97%in the control group,the difference being significant(P<0.05).After treatment,the scores of VAS and WOMAC in both groups were decreased when compared with those before treatment(P<0.05),and the above two scores in the study group were lower than those in the control group(P<0.05).After treatment,the levels of TNF-αand IL-6 in serum in both groups were significantly decreased when compared with those before treatment(P<0.05),and the above two levels in the study group were lower than those in the control group(P<0.05).During the treatment,there was no obvious adverse reaction in both groups,and no symptoms of skin scald and allergy in the study group,indicating great safety.Conclusion:Fire-dragon moxibustion with Wenbi powder can significantly relieve pain,effectively improve joint function and reduce inflammation.
