光动力治疗脉冲染料激光抵抗的鲜红斑痣的疗效观察

摘　　要：目的观察海姆泊芬光动力治疗儿童脉冲染料激光抵抗鲜红斑痣患儿的疗效和不良反应。方法纳入脉冲染料激光治疗≥5次仍然无效的患者27例,静脉注射海姆泊芬(血卟啉单甲醚,5mg/kg),随后让患处暴露于532 nm LED绿光下,照射功率密度为75~85 mW/cm^(2),照射时间为15~20 min。结果27例患者经海姆泊芬光动力治疗后,11例(40.74%)达到全愈(消退≥90%),12例(44.44%)为显效(消退60%~90%),3例(11.11%)为有效(消退20%~60%),1例(9.76%)为无效(消退<20%),总有效率为96.30%,未观察到明显的全身性不良反应。局部的短暂不良反应包括肿胀、紫癜、结痂和色素沉着,无需治疗即可消失。结论海姆泊芬光动力治疗儿童脉冲染料激光治疗抵抗的鲜红斑痣安全有效。Objective To observe the efficacy and adverse reactions of Hemporfin photodynamic therapy for children with port-wine stains resistant to pulsed dye laser therapy.Methods Twenty-seven patients who were still ineffective after≥5 pulsed dye laser treatments were included.Hemoporfin(hemoporphyrin monomethyl ether,5 mg/kg)was intravenously injected,and then the affected area was exposed to 532 nm LED green light with a power density of 75-85 mW/cm^(2).The irradiation time was 15-20 minutes.Results After photodynamic therapy of hemporfin in 27 patients,11 patients(40.74%)achieved basic recovery(regression≥90%),12 patients(44.44%)were markedly effective(regression<90%,≥60%),3 cases(11.11%)were effective(regression<60%,≥20%),the 1 case(9.76%)was ineffective(regression<20%),with the total effective rate of 96.30%.No obvious systemic adverse reactions were observed.Local transient adverse effects include swelling,purpura,crusting,and hyperpigmentation,which disappeared without treatment.Conclusion Hemporfin photodynamic therapy is safe and effective in children with port-wine stains resistant to pulsed dye laser therapy.
