摘 要:目的 应用二维斑点追踪技术评估飞行员急性正加速度(+Gz)暴露后左室心肌功能的变化。方法 共纳入53例飞行员参与+6Gz载人离心机实验,采用常规联合二维斑点追踪技术评估飞行员急性+6Gz暴露前、后左室常规参数及左室各平面纵向应变参数的变化。结果 与静息状态相比,+6Gz暴露后飞行员的血压、心率升高(P<0.001);LVIDd、LVIDs、E/A、LVEDV、LVESV显著减低(P<0.05);SV、LVEF、LVFS、IVSd、LVPWd、心尖四腔收缩峰值应变(GLPS-A4C)无统计学意义(P>0.05);左室整体收缩峰值应变(GLPS-AVG)、心尖两腔收缩峰值应变(GLPS-A2C)、左室长轴收缩峰值应变(GLPS-LAX)绝对值显著减低,差异有统计学意义(P<0.05)。结论 二维斑点追踪技术能灵敏检测急性+Gz暴露后飞行员左室收缩功能的变化。Objective Two-dimensional speckle tracking technique was used to assess the changes of left ventricular myocardial function in pilots after acute positive acceleration(+Gz) exposure. Methods A total of 53 pilots were included in the +6 Gz human centrifuge test. The left ventricular routine parameters and left ventricular longitudinal strain parameters before and after acute +6 Gz exposure were evaluated by conventional echocardiography combined two-dimensional speckle tracking technique. Results Compared with resting state, the blood pressure and heart rate of pilots increased immediately after +6 Gz exposure(P<0.001). LVIDd, LVIDs, E/A,LVEDV,LVESV decreased significantly(P<0.05). There were no significant difference in SV,LVEF,LVFS,IVSd, LVPWd and apical four-chamber global systolic peak strain(GLPS-A4 C)(P>0.05). The absolute values of left ventricular global peak systolic strain(GLPS-AVG), apical two-chamber global peak systolic strain(GLPS-A2 C) and left ventricular long-axis peak systolic strain(GLPS-LAX) decreased(P<0.05). Conclusions Two-dimensional speckle tracking technique can sensitively detect the changes of left ventricular systolic function in pilots after acute +Gz exposure.
