超声造影在脾脏局灶性病变的诊断价值

摘　　要：目的 评估超声造影在脾脏局灶性病变的诊断及鉴别诊断价值。方法 回顾性分析79例患者共130个脾脏局灶性病变的超声造影声像特征。结果 130例病变中,良性90例,恶性40例。其中血管瘤57例,梗死10例,囊肿7例,淋巴管瘤8例,血肿5例,转移灶28例,淋巴瘤9例,血管肉瘤3例,畸胎瘤、副脾、挫伤各1例。超声显示75%的恶性病灶在动脉期表现为低增强,实质期均快速廓清呈低强化;良性病灶可表现为无强化、高强化、等强化及低强化,实质期为缓慢或同步廓清。超声造影和常规超声对脾脏局灶性病变良恶性的诊断灵敏度、特异度、准确度分别为90%、98.9%、96.2%及75%、73%、73.8%。结论 超声造影可以为脾脏局灶性病变的诊断及鉴别诊断提供有价值的信息。Objective To evaluate the value of ontrast-enhanced in diagnosis and differential diagnosis of spleen focal lesions. Methods The contrast-enhanced ultrasonographic features of 130 spleen focal lesions in 79 patients were retrospectively analyzed. Results There were 90 benign lesions and 40 malignant lesions, including 57 hemangiomas, 28 metastases, ten infarctions, nine lymphomas, eight lymphangiomas, seven cysts, five hematomas, three angiosarcomas, one teratoma, one accessory spleen and one contusion. In CEUS, 75% of malignant lesions showed hypoenhancement in the arterial phase, and all lesions were fast washout in the parenchymal phase. While benign lesions showed no enhancement, hyper-/iso-/hypo enhancement in the arterial phase and slow or synchronous washout in the parenchymal phase. The sensitivity, specificity and accuracy of diagnosis for benign and malignant FSLs were 98.9%, 90% and 96.2% for CEUS and 75%, 73% and 73.8% respectively. Conclusions CEUS can provide valuable information for the diagnosis and differential diagnosis of FSLs.
