黄葵胶囊联合前列地尔及羟苯磺酸钙治疗药物性肾损伤的临床疗效

摘　　要：目的 观察黄葵胶囊联合前列地尔及羟苯磺酸钙治疗药物性肾损伤的临床疗效。方法 选取2017年10月-2020年10月丽江市人民医院收治的药物性肾损伤患者92例,按照随机数字表法分为对照组与观察组,各46例。对照组予以前列地尔联合羟苯磺酸钙,观察组在对照组基础上予以黄葵胶囊,2组均治疗2个月。比较2组临床疗效,治疗前后中医症候积分、肾功能指标[胱抑素C(Cys C)、血肌酐(Scr)、血尿素氮(BUN)、24 h尿蛋白定量],临床症状缓解时间及不良反应。结果 治疗2个月,观察组总有效率为97.83%,高于对照组的82.61%(χ^(2)=4.434,P=0.035)。治疗2个月后,2组主症积分、次症积分、舌脉积分及总分低于治疗前,且观察组低于对照组(P <0.01)。治疗2个月后,2组Cys C、Scr、BUN水平及24 h尿蛋白定量低于治疗前,且观察组低于对照组(P <0.01)。观察组食欲不振、腰酸、夜尿增多、水肿症状缓解时间短于对照组(P <0.01)。观察组不良反应发生率为23.91%,与对照组的17.39%比较,差异无统计学意义(χ^(2)=0.597,P=0.440)。结论 黄葵胶囊联合前列地尔及羟苯磺酸钙治疗药物性肾损伤患者的临床疗效确切,可有效改善患者肾功能,缓解临床症状,且不明显增加不良反应。Objective To observe clinical efficacy of Huangkui capsule combined with alprostadil and calcium dobesilate in the treatment of drug-induced renal injury. Methods A total of 92 cases of patients with drug-induced renal injury were selected from October 2017 to October 2020 in Lijiang People’ s Hospital,which were divided into control group and observation group according to the random number table method,with 46 cases in each group. The control group was given alprostadil combined with calcium dobesilate,and the observation group was given Huangkui capsule on the basis of the control group. Both groups were treated for 2 months. Clinical efficacy,TCM syndrome score and renal function index [serum cystatin C( Cys C),serum creatinine( Scr),blood urea nitrogen( BUN),24 h urine protein quantification]before and after treatment,clinical symptoms relief time and adverse reactions were compared between the two groups. Results After 2 months of treatment,the total effective rate in the observation group was 97. 83%,higher than 82. 61% in the control group( χ^(2)= 4. 434,P = 0. 035). After 2 months of treatment,the main symptom score,secondary symptom score,tongue and pulse score and total score of the two groups were lower than those before treatment,and the observation group were lower than those of the control group( P < 0. 01). After 2 months of treatment,the levels of Cys C,Scr,BUN and 24 h urine protein quantification in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group( P < 0. 01). The relief time of anorexia,lumbar acid,nocturia and edema in the observation group were shorter than those in the control group( P < 0. 01). The incidence of adverse reactions in the observation group was 23. 91%,which was not statistically significant compared with 17. 39% in the control group( χ^(2)= 0. 597,P = 0. 440).
