观察浮针结合中医穴位按摩对老年膝骨性关节炎伴失眠患者骨关节功能和睡眠质量的影响

摘　　要：目的:观察浮针结合中医穴位按摩对老年膝骨性关节炎(KOA)伴失眠患者骨关节功能和睡眠质量的影响。方法:选取2020年1月至2021年10月开平市中心医院中医骨科收治的老年KOA伴失眠患者76例作为研究对象,按照随机数字表法分为观察组和对照组,每组38例。对照组给予中医穴位按摩进行治疗,观察组在对照组基础上联合给予浮针治疗,比较2组患者的治疗效果,采用视觉模拟评分法(VAS)比较2组膝关节疼痛改善情况,采用膝关节评分量表(HSS)评估患者骨关节功能,采用匹兹堡睡眠质量指数(PSQI)评估患者睡眠质量,比较2组患者炎症介质水平。结果:治疗后,观察组VAS评分、PSQI评分、IL-6、IL-1β和TNF-α水平均显著低于对照组,观察组临床治疗有效率高于对照组,HSS评分显著高于对照组,2组比较差异均有统计学意义(均P<0.05)。结论:采用浮针结合中医穴位按摩治疗老年KOA伴失眠患者可明显改善骨关节功能,提高睡眠质量,疗效明显。Objective:To observe the effects of floating acupuncture combined with traditional Chinese medicine acupoint massage on bone and joint function and sleep quality of elderly knee osteoarthritis(KOA)patients with insomnia.Methods:A total of 76 elderly patients with KOA and insomnia admitted to the department of orthopedics of traditional Chinese medicine in Kaiping Central Hospital from January 2020 to October 2021 were selected as the research objects,and were divided into observation group and control group according to the random number table method,with 38 cases in each group.The control group was given traditional Chinese medicine acupoint massage for treatment,and the observation group was given floating acupuncture on the basis of the control group.The treatment effects of the two groups were compared.Visual analogue scale(VAS)was used to compare the improvement of knee pain between the 2 groups.Hospital for Special Surgery Knee Score(HSS)was used to compare the bone and joint function,and Pittsburgh Sleep Quality Index(PSQI)were used to evaluate the sleep quality of patients,and the levels of inflammatory factors in 2 groups were compared at the same time.Results:After treatment,VAS score,PSQI score,IL-6,IL-1βand TNF-αlevels in the observation group were significantly lower than those of the control group,clinical effective rate in the observation group was higher than the control group,HSS score was significantly hinger than the control group,the differences were statistically significant(Ps<0.05).Conclusion:Floating acupuncture combined with acupoint massage of traditional Chinese medicine can significantly improve bone and joint function and sleep quality in elderly patients with KOA and insomnia.
