COOK宫颈扩张球囊联合人工破膜及缩宫素静脉点滴在足月妊娠引产中的临床效果

摘　　要：目的探讨COOK宫颈扩张球囊联合人工破膜及缩宫素静脉点滴在足月妊娠引产中的临床效果。方法选择2019年12月至2021年10月足月妊娠于武汉真爱妇产医院妇产科计划分娩引产的140例产妇为研究对象,根据治疗方式不同分为观察组和对照组,每组各70例。对照组单纯采用缩宫素促宫颈成熟引产,观察组在对照组的基础上采用COOK宫颈扩张球囊联合人工破膜,比较两组的引产效果。结果观察组的第一产程、第二产程和总产程短于对照组(P<0.05),但两组的第三产程比较,差异无统计学意义(P>0.05)。观察组静滴缩宫素临产时间短于对照组;观察组缩宫素引产至分娩时间、住院时间短于对照组,Bishop评分高于对照组,差异均有统计学意义(P<0.05)。两组胎儿Apgar评分比较,差异无统计学意义(P>0.05)。观察组的分娩方式优于对照组,且观察组不良母婴结局总发生率低于对照组,差异均有统计学意义(P<0.05)。两组的各类并发症总发生率比较,差异无统计学意义(P>0.05)。结论COOK宫颈扩张球囊与人工破膜及缩宫素联合应用于足月妊娠引产孕妇可以有效缩短产程,减少并发症的发生,提高产妇自然分娩率,改善母婴结局,临床干预效果显著。Objective To discuss the clinical effect of Cook cervical ripening balloon combined with artificial membrane rupture and Oxytocin intravenous dripping in the induced labor of full-term pregnancy women.Methods A total of 140 full-term pregnancy women who planned to undergo labor induction in Department of Gynecology and Obstetrics,Wuhan Zhenai Women Hospital from December 2019 to October 2021 were selected as research subjects.According to different treatment modes,women were divided into the observation group(n=70)and the control group(n=70).The control group just took the Oxytocin,on the basis of the former,the observation group took the Cook cervical ripening balloon and artificial membrane rupture.Induced labor outcomes were compared.Results The first,second and overall labor stages in the observation group were shorter than those in the control group(P<0.05).The third labor stage between groups was not significantly different(P>0.05).The labor time of Oxytocin intravenous dripping in the observation group was shorter than that in the control group;the Oxytocin dripping to delivery time,and hospital stays in the observation group were shorter than those in the control group,Bishop score in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).There was no significant difference in fetal Apgar score between the two groups(P>0.05).The delivery mode of the observation group was better than that of the control group,and the total incidence of adverse maternal and infant outcomes in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Cook cervical ripening balloon combined with artificial membrane rupture and Oxytocin intravenous dripping can shorten the labor stage,reduce the clinical complications,and improve the spontaneous labor rates and maternal and infant outcomes,wit
