复方活血灵汤治疗股骨颈骨折术后深静脉血栓形成的临床效果

摘　　要：目的 探讨复方活血灵汤治疗股骨颈骨折术后深静脉血栓形成的效果。方法 选取2019年6月至2021年6月四川省遂宁市中医院收治的股骨颈骨折术后深静脉血栓形成患者82例,按照随机数字表法将其分为对照组和观察组,每组41例。对照组给予常规西药治疗,观察组在对照组的基础上联合复方活血灵汤治疗,两组均治疗2周。治疗后进行临床疗效评价。比较两组治疗前后的大腿和小腿周径差值、血液流变学指标和炎症因子水平。结果 观察组的临床疗效优于对照组(P <0.05)。治疗后,两组大腿和小腿周径差值较治疗前减小,且观察组小于对照组(P <0.05)。两组血浆黏度、全血高切黏度、血小板黏附率、白细胞介素-6、肿瘤坏死因子-α水平较治疗前降低,且观察组低于对照组(P <0.05)。两组血清白细胞介素-10水平较治疗前升高,且观察组高于对照组(P <0.05)。结论 在常规西药治疗基础上应用复方活血灵汤治疗股骨颈骨折术后深静脉血栓形成效果显著,能够明显减轻症状,改善血液高凝状态,抑制炎症反应。Objective To investigate the effect of compound Huoxueling Decoction in the treatment of deep venous thrombosis after femoral neck fracture. Methods A total of 82 patients with deep venous thrombosis after femoral neck fracture treated in Suining Municipal Hospital of Traditional Chinese Medicine of Sichuan Province from June 2019 to June 2021 were selected, and they were divided into control group and observation group according to random number table method, with 41 cases in each group. The control group was treated with conventional Western medicine, and the observation group was treated with compound Huoxueling Decoction on the basis of the control group. Both groups were treated for two weeks. The clinical efficacy was evaluated after treatment. The difference of thigh and calf circumference, hemorheology indexes, and inflammatory factor levels were compared between the two groups before and after treatment. Results The clinical effect of the observation group was better than the control group(P < 0.05). After treatment, the difference of thigh and calf circumference between the two groups was decreased compared with that before treatment, and those in the observation group were smaller than those in the control group(P < 0.05). The levels of plasma viscosity, whole blood hypershear viscosity, platelet adhesion rate, interleukin-6, and tumor necrosis factor-α in two groups were lower than those before treatment, and those in the observation group were lower than those in the control group(P < 0.05). The level of serum interleukin-10 in both groups was higher than that before treatment, and that in the observation group was higher than that in the control group(P < 0.05). Conclusion On the basis of conventional Western medicine treatment, the application of compound Huoxueling Decoction in the treatment of deep venous thrombosis after femoral neck fracture has a significant effect, which can significantly reduce symptoms, improve blood hypercoagulability and inhibit inflammatory reaction.
