摘　　要：目的:分析嗜酸性粒细胞增多症(HES)累及心脏的临床表现及超声心动图特征,并评价超声心动图对HES的诊断价值。方法:回顾性分析我院63例确诊为嗜酸性粒细胞增多症的患者的临床资料及超声心动图特征并对其进行分析。结果:63例患者中,心脏受累36例(57.1%)。36例患者中累及心血管系统的超声心动图表现主要有Loeffler心内膜炎合并心室内血栓形成6例(16.7%)、冠状动脉粥样硬化性心脏病22例(61.1%)、肺栓塞4例(11.1%)及机械瓣置换术后短期内机械瓣故障2例(5.6%)。结论:Loeffler心内膜炎合并心室内血栓形成是HES特征性超声心动图表现。HES可引起急性胸痛症状,如心肌梗死和肺栓塞,超声心动图检查可以较准确地帮助临床进行诊断,并对心脏瓣膜置换术后患者提示预后。Objective:To analyze the clinical manifestation and echocardiographic features of Hypereosinophilic syndrome(HES)involving the heart,and to evaluate the diagnostic value of echocardiography in HES.Methods:The clinical findings and echocardiographic features were retrospectively analyzed in 63 patients with a clinical diagnosis of hypereosinophilic syndrome.Results:Among the 63 patients,36patients(57.1%)had cardiac involvement.Among the 36 patients,the main clinical manifestations involving the cardiovascular system included Loeffler endocarditis with intraventricular thrombosis in 6 cases(16.7%),coronary heart disease in 22 cases(61.1%),pulmonary embolism in 4 cases(11.1%),and mechanical valve failure in the short term after mechanical valve replacement in 2 cases(5.6%).Conclusions:The characteristic echocardiographic findings were Loeffler endocarditis with intraventricular thrombosis.HES can cause acute chest pain symptoms,such as acute myocardial infarction and pulmonary embolism.Echocardiography can be used to diagnose HES accurately and predict prognosis after cardiac valve replacement.
