摘  要:目的:探讨2型糖尿病(T2DM)患者肠道菌群水平与白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、胰岛素抵抗指数(HOMA-IR)水平的相关性。方法:选取126例T2DM患者为研究对象,依据糖化血红蛋白(HbA1c)水平将其分为血糖控制良好组75例(HbA1c<7%)、血糖控制不良组51例(HbA1c≥7%),同时选取同期健康体检者120名为对照组。比较三组肠道菌群、HOMA-IR、IL-6和TNF-α水平,采用Pearson相关分析肠道菌群水平与HOMA-IR、IL-6、TNF-α水平的相关性。结果:血糖控制不良组肠杆菌、肠球菌水平高于血糖控制良好组、对照组,且血糖控制良好组高于对照组,血糖控制不良组双歧杆菌、拟杆菌、乳酸杆菌水平均低于血糖控制良好组、对照组,且血糖控制良好组低于对照组,差异有统计学意义(P<0.05);血糖控制不良组HOMA-IR、IL-6、TNF-α水平均高于血糖控制良好组、对照组,且血糖控制良好组高于对照组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,肠杆菌、肠球菌水平与HOMA-IR、IL-6、TNF-α水平呈正相关(r>0,P<0.05),双歧杆菌、拟杆菌、乳酸杆菌水平与HOMA-IR、IL-6、TNF-α水平呈负相关(r<0,P<0.05)。结论:T2DM患者肠道菌群水平紊乱,其水平与IL-6、TNF-α、HOMA-IR水平存在相关性。Objective:To explore correlations among intestinal flor levels with interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and insulin resistance index(HOMA-IR)levels in patients with type 2 diabetes mellitus(T2DM).Methods:126 T2DM patients were selected as the research objects.According to the level of glycosylated hemoglobin(HbA1c),these patients with T2DM were divided into good blood glucose control group(75 cases,HbA1c<7%)and poor blood glucose control group(51 cases,HbA1c≥7%).At the same time,120 healthy subjects were selected as the control group.The levels of intestinal flora,HOMA-IR,IL-6 and TNF-αwere compared among the three groups.Pearson correlation was used to analyze the correlations among intestinal flor levels and the levels of HOMA-IR,IL-6 and TNF-α.Results:The levels of Enterobacter and Enterococcus in the poor blood glucose control group were higher than those in the good blood glucose control group and the control group,those of the good blood glucose control group were higher than those of the control group;the levels of Bifidobacteria,Bacteroides and Lactobacilli in the poor blood glucose control group were lower than those in the good blood glucose control group and the control group,those of the good blood glucose control group were lower than those of the control group;and the differences were statistically significant(P<0.05).The levels of HOMA-IR,IL-6 and TNF-αin the poor blood glucose control group were higher than those in the good blood glucose control group and the control group,those of the good blood glucose control group were higher than those of the control group,and the differences were statistically significant(P<0.05).The Pearson correlation analysis showed that the levels of Enterobacter and Enterococcus were positively correlated with the levels of HOMA-IR,IL-6 and TNF-α(r>0,P<0.05),and he levels of Bifidobacteria,Bacteroides and Lactobacilli were negatively correlated with the levels of HOMA-IR,IL-6 and TNF-α(r<0,P<0.05).Conclusions:The T2DM patients have intestinal flo
