支撑喉镜下低温等离子刀切除术与电刀切除术治疗会厌囊肿患者的效果比较

摘　　要：目的:比较支撑喉镜下低温等离子刀切除术与电刀切除术治疗会厌囊肿患者的效果。方法:选取80例会厌囊肿患者为研究对象,按照随机数字表法分为对照组和观察组各40例。对照组采用支撑喉镜下电刀切除术治疗,观察组采用支撑喉镜下低温等离子刀切除术治疗。比较两组手术相关指标(手术时间、伤口愈合时间、术中出血量和术后咽痛持续时间)水平、疼痛[视觉模拟评分法(VAS)]评分、术后伪膜消失率、复发率和并发症发生率。结果:观察组手术时间、伤口愈合时间和术后咽痛持续时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后1、2、4 d,观察组VAS评分均显著低于对照组,差异有统计学意义(P<0.05);术后10、15、20 d,观察组伪膜消失率明显高于对照组,差异均有统计学意义(P<0.05);观察组并发症发生率、复发率与对照组比较,差异均无统计学意义(P>0.05)。结论:支撑喉镜下低温等离子刀切除术治疗会厌囊肿患者可改善手术相关指标水平,降低术后VAS评分,以及提高术后伪膜消失率,优于支撑喉镜下电刀切除术治疗效果。Objective:To compare effects of low temperature plasma knife resection and electrosurgical excision under suspension laryngoscope in treatment of patients with epiglottis cysts.Methods:80 patients with epiglottis cysts were selected as the research objects,and were divided into control group and observation group according to the random number table method,40 cases in each group.The control group was treated with electrosurgical excision under the suspension laryngoscope,while the observation group was treated with low temperature plasma knife excision under the suspension laryngoscope.The levels of surgery-related indicators(operation completion time,wound healing time,intraoperative blood loss,and postoperative sore throat duration time),the pain level[visual analog scale(VAS)],the postoperative pseudomembrane disappearance rate,the recurrence rate and the incidence of complications were compared between the two groups.Results:The operation time,the wound healing time and the postoperative sore throat time in the observation group were significantly shorter than those in the control group;the intraoperative blood loss was less than that in the control group;and the differences were statistically significant(P<0.05).1,2,and 4 days after the surgery,the VAS scores of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).10,15 and 20 days after the surgery,the pseudomembrane disappearance rates in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).However,there were no significant differences in the incidence of complications and the recurrence rate between the observation group and the control group(P>0.05).Conclusions:Low temperature plasma knife resection under the suspension laryngoscope in the treatment of the patients with epiglottis cysts can improve the levels of surgery-related indicators,reduce the postoperative VAS score,and increase
