雌激素联合醋酸甲羟孕酮治疗围绝经期功能失调性子宫出血患者的效果

摘　　要：目的:观察雌激素联合醋酸甲羟孕酮治疗围绝经期功能失调性子宫出血患者的效果。方法:选取124例围绝经期功能失调性子宫出血患者为研究对象,按随机数字表法分为对照组和观察组各62例。对照组给予醋酸甲羟孕酮治疗,观察组在对照组基础上给予结合雌激素片治疗。比较两组临床疗效,治疗前后血清性激素指标[孕酮(P)、卵泡刺激素(FSH)、雌二醇(E_(2))及黄体生成素(LH)]水平、子宫内膜厚度、血红蛋白水平,以及不良反应发生率。结果:观察组治疗总有效率为96.77%(60/62),高于对照组的79.03%(49/62),差异有统计学意义(P<0.05);治疗后,观察组血红蛋白水平高于对照组,P、FSH、E_(2)、LH水平和子宫内膜厚度均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:结合雌激素片联合醋酸甲羟孕酮治疗围绝经期功能失调性子宫出血患者可提高治疗总有效率和血红蛋白水平,降低血清性激素指标水平和子宫内膜厚度,效果优于单纯醋酸甲羟孕酮治疗。Objective:To observe effects of Estrogen combined with Medroxyprogesterone acetate the treatment of patients with perimenopausal dysfunctional uterine bleeding.Methods:124 patients with perimenopausal dysfunctional uterine bleeding were selected as the research objects,and were divided into control group and observation group according to the random number table method,62 cases in each group.The control group was treated with Medroxyprogesterone acetate,while the observation group was treated with Estrogen on the basis of that of the control group.The clinical effects,the serum sex hormone index levels[progesterone(P),follicle-stimulating hormone(FSH),estradiol(E_(2))and luteinizing hormone(LH)],the endometrial thickness,the hemoglobin level and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 96.77%(60/62),which was higher than that the control group of 79.03%(49/62),and the difference was statistically significant(P<0.05).After the treatment,the hemoglobin level in the observation group was higher than that in the control group;the levels of P,FSH,E_(2),LH and endometrial thickness were lower than those in the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Conjugated estrogen tablets combined with Medroxyprogesterone acetate in the treatment of THE patients with perimenopausal dysfunctional uterine bleeding can improve the total effective rate and the hemoglobin level and reduce the levels of serum sex hormone indexes and the endometrial thickness.Moreover,it is superior to single Medroxyprogesterone acetate treatment.
