二代测序技术在社区获得性鹦鹉热衣原体肺炎诊断中的应用价值

摘　　要：目的探讨二代测序技术对社区获得性鹦鹉热衣原体肺炎的诊断价值。方法4例社区获得性鹦鹉热衣原体肺炎患者作为研究对象,均经二代测序技术检测进行诊断。分析患者的临床特点及检验结果、临床治疗方案和疗效。结果4例患者均为52~66岁的老年家庭妇女,典型表现(高热、咳嗽)、衣原体抗体免疫球蛋白M(IgM)检测结果及痰培养均为阴性;胸部CT以单侧肺叶实变为主。4例患者可见鹦鹉热衣原体种属基因检出的序列数为1~61个,中位检出序列数为24个;二代测序技术阳性检出率为75.00%(3/4)。4例患者均经对症治疗后病情好转。结论二代测序技术诊断社区获得性鹦鹉热衣原体肺炎,其方法更快速、准确、全面,可作为社区获得性鹦鹉热衣原体肺炎的诊断方法之一。Objective To discuss the value of second-generation sequencing technology in the diagnosis of community-acquired Chlamydia psittaci pneumonia.Methods 4 patients with community-acquired Chlamydia psittaci pneumonia were selected as the research subjects,and all of them were diagnosed by second-generation sequencing technology.The clinical characteristics,test results,clinical regimens and efficacy of the patients were analyzed.Results All 4 patients were elderly housewives aged 52-66 years with typical manifestations(high fever and cough),negative immunoglobulin M(IgM)test results for chlamydial antibodies and sputum culture;chest CT was dominated by solid unilateral lung lobes.The number of detected sequences of Chlamydia psittaci species genes in 4 patients ranged from 1 to 61,with a median number of 24 sequences detected.The positive detection rate by second-generation sequencing technology was 75.00%(3/4).All 4 patients improved after symptomatic treatment.Conclusion The second-generation sequencing technology for the diagnosis of community-acquired Chlamydia psittaci pneumonia is more rapid,accurate and comprehensive,and can be used as one of the diagnostic methods for community-acquired Chlamydia psittaci pneumonia.
