复元胶囊联合醒脑开窍针刺治疗血管性痴呆的疗效及对血管内皮生长因子、同型半胱氨酸和脑源性神经营养因子的影响

摘　　要：目的分析复元胶囊联合醒脑开窍针刺治疗血管性痴呆(Vascular dementia,VD)的疗效及对血管内皮生长因子(Vascular endothelial growth factor,VEGF)、同型半胱氨酸(Homocysteine,Hcy)、脑源性神经营养因子(Brain-derived neurotrophic factor,BDNF)水平的影响。方法选取2019年1月—2020年9月期间河北省衡水市中医医院收治的119例VD患者,按随机数字表法分为对照组59例、观察组60例。对照组予以予盐酸多奈哌齐、尼莫地平及阿司匹林口服治疗,观察组在对照组基础上予以复元胶囊联合醒脑开窍针刺治疗。两组患者均持续治疗8周后,观察比较两组患者治疗后临床疗效,比较两组患者治疗前后的简易智力状况量表(Mini-mental state examination,MMSE)、改良Barthel指数(Modified barthel index,MBI)评分、血清学指标(VEGF、Hcy、BDNF水平)及不良反应情况。结果治疗后观察组总有效率87.93%(51/58)高于对照组72.41%(42/58),差异有统计学意义(P<0.05)。治疗后两组患者MMSE、MBI评分均较治疗前明显升高,差异有统计学意义(P<0.05);且观察组均高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清VEGF、BDNF水平均较治疗前明显升高,Hcy水平均较治疗前降低,差异有统计学意义(P<0.05);且观察组血清VEGF、BDNF水平均明显高于对照组,Hcy水平明显低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论复元胶囊联合醒脑开窍针刺可有效改善VD患者的智力认知功能、生活自理能力及血清VEGF、Hcy、BDNF水平,且安全性高,值得临床推广运用。Objective To analyze the clinical efficacy of Fuyuan Capsules combined with Xingnao Kaiqiao acupuncture(acupuncture for inducing resuscitation)on vascular dementia(VD)and theeffects on serum vascular endothelial growth factor(VEGF),homocysteine(Hcy),and brain-derived neurotrophic factor(BDNF).Methods A total of 119 patients with VD treated in Hengshui Hospital of Traditional Chinese Medicine from January 2019 to September 2020 were divided into a control group(59)and an observation group(60)according to a random number table.All patients were treated with donepezil hydrochloride,nimodipine,and aspirin,while those in the observation group received additional Fuyuan Capsules combined with Xingnao Kaiqiao acupuncture.All patients were treated for eight weeks.The clinical efficacies of the two groups after treatment wereobserved.The Mini-mental State Examination(MMSE)scores,Modified Barthel Index(MBI)scores,and the levels of VEGF,Hcy,and BDNF of the two groups were compared before and after treatment.The adverse events during the treatment of the two groups were recorded.Results The total effective rate of the observation group was 87.93%(51/58),higher than 72.41%(42/58)of the control group(P<0.05).After treatment,the MMSE and MBI scores of the two groups were higher than those before treatment(P<0.05),and the observation group was higher than the control group(P<0.05).The post-treatment serum VEGF and BDNF levels increased and the serum Hcy levelsdecreased in both groups as compared with those before treatment(P<0.05).The serum VEGF and BDNF levels were higher and the serum Hcy level was lower in the observation group than those in the control group(P<0.05).There wasno significant difference in the incidence of adverse events between the two groups during treatment(P>0.05).Conclusion Fuyuan Capsules combined with Xingnao Kaiqiao acupuncture can effectively improve the cognitive function,self-care ability,and serum VEGF,Hcy,BDNF levels of patients with VD,withgood safety,which is worthy of clinical application.
