加味二陈汤联合阿托伐他汀对痰湿阻遏型高脂血症患者炎症因子、尿酸、颈动脉斑块的影响

摘　　要：目的:观察加味二陈汤联合阿托伐他汀治疗痰浊阻遏型高脂血症患者炎症因子、尿酸、颈动脉斑块的影响。方法:将60例痰浊阻遏型高脂血症患者随机分为对照组和观察组,每组30例,对照组口服阿托伐他汀片,观察组在对照组的基础上服用加味二陈汤,治疗12周后,观察治疗前后两组患者炎症因子、尿酸、颈动脉最大斑块面积、颈动脉内中膜层厚度(intima media thickness,IMT)以及不良反应发生情况。结果:两组患者治疗后白介素6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor–α,TNF-α)和C反应蛋白(c-reactive protein,CRP)均较治疗前明显下降(P<0.05),观察组炎症因子改善明显于对照组(P<0.05)。治疗后两组患者尿酸均有下降,观察组尿酸下降更明显,与对照组比较,差异有统计学意义(P<0.05)。治疗后两组颈动脉最大斑块面积均有缩小,观察组缩小明显,与对照组比较,差异有统计学意义(P<0.05)。治疗后两组颈动脉IMT较治疗前降低但差异无统计学意义(P>0.05)。两组均未发生与药物相关的严重不良反应。结论:加味二陈汤联合阿托伐他汀能有效地降低痰浊阻遏型高脂血症患者炎症因子和尿酸水平,缩小斑块面积,且无明显不良反应。Objective:To observe the effects of Jiawei Erchen Decoction combined with Atorvastatin on inflammatory factors,uric acid and carotid plaque in hyperlipidemia patients with phlegm turbidity suppressed syndrome.Methods:60 hyperlipidemia patients with phlegm turbidity repressed were randomly divided into control group and observation group,with 30 cases in each group.The control group took Atorvastatin tablets orally,and the observation group took Jiawei Erchen Decoction on the basis of the control group.After 12 weeks of treatment,the inflammatory factors,uric acid,maximum carotid plaque area,carotid intima media thickness(IMT)and adverse reactions were observed before and after treatment.Results:IL-6,TNF-αand CRP levels in the two groups after treatment were significantly lower than those before treatment(P<0.05),and the improvement of inflammatory factors in the observation group was better than those in the control group(P<0.05).After treatment,the area of the largest carotid plaque in the two groups decreased,and the observation group decreased significantly.Compared with the control group,the difference was statistically significant(P<0.05).After treatment,the carotid IMT in the two groups decreased compared with that before treatment,but the difference was not statistically significant.Conclusion:Jiawei Erchen Decoction combined with Atorvastatin can effectively reduce the levels of inflammatory factors and uric acid,reduce the plaque area and have no obvious adverse reactions.
