摘  要:目的探讨妊娠期糖尿病(GDM)患者血清晚期糖基化终末产物受体(RAGE)、核因子-κB(NF-κB)表达水平及其与胰岛素抵抗(IR)的关系。方法选取2020年1月至2021年6月在海口市妇幼保健院建档产检、行口服葡萄糖耐量试验(OGTT)诊断为GDM的108例患者作为GDM组,并选取同期在同一医院建档产检、OGTT正常的120例孕妇作为对照组。采用全自动生化分析仪检测所有研究对象的空腹血糖(FPG)、空腹胰岛素水平(FINS),并计算胰岛素抵抗指数(HOMA-IR);根据HOMA-IR将GDM患者分为非IR组(n=37,HOMA-IR<1.66)和IR组(n=71,HOMA-IR≥1.66)。采用酶联免疫吸附试验(ELISA)法检测所有研究对象血清RAGE、NF-κB的表达水平;采用Pearson法分析GDM患者血清RAGE、NF-κB的相关性及二者与血清HOMA-IR、FPG、FINS的相关性;采用受试者工作特征(ROC)曲线分析血清RAGE、NF-κB的表达水平预测GDM患者IR的价值;采用多元线性回归分析影响患者HOMA-IR的因素。结果与对照组比较,GDM组血清FPG、FINS、HOMA-IR、RAGE、NF-κB表达水平均显著升高(P<0.05);与非IR组比较,IR组患者血清RAGE、NF-κB表达水平均显著升高(P<0.05)。Pearson法表明,GDM患者血清RAGE、NF-κB的表达呈正相关(r=0.664,P<0.05);GDM患者血清RAGE、NF-κB的表达与HOMA-IR、FPG、FINS均呈正相关(P<0.05)。血清RAGE联合NF-κB的表达水平预测GDM患者IR的曲线下面积明显高于其单独预测(P<0.05);血清RAGE、NF-κB水平是HOMA-IR的影响因素(P<0.05)。结论GDM患者血清RAGE、NF-κB的表达水平显著升高,且与患者HOMA-IR呈正相关,其可能是GDM研究和治疗的潜在靶点。Objective To investigate the expression levels of receptor for advanced glycation end-product(RAGE)and nuclear factor-κB(NF-κB)in serum of patients with gestational diabetes mellitus(GDM)and their relationship with insulin resistance(IR).Methods From January 2020 to June 2021,108 cases of pregnant women who were filed in Haikou Maternal and Child Health Hospital for prenatal examination and diagnosed with GDM by oral glucose tolerance test(OGTT)were selected as the GDM group,and 120 cases of pregnant women with normal OGTT in the same hospital during the same period were selected as the control group.The automatic biochemical analyzer was used to detect fasting blood glucose(FPG)and fasting insulin levels(FINS)of all subjects,and calculate the insulin resistance index(HOMA-IR);according to HOMA-IR value,patients with GDM were divided into non-IR group(n=37,HOMA-IR<1.66)and IR group(n=71,HOMA-IR≥1.66).Enzyme-linked immunosorbent assay(ELISA)test was used to detect the expression levels of all subjects serum RAGE and NF-κB;Pearson method was used to analyze the correlation between serum RAGE and NF-κB in patients with GDM and the correlation between the two and HOMA-IR,FPG,FINS;receiver operating characteristic(ROC)curve analysis of serum RAGE and NF-κB expression levels to predict IR in patients with GDM;multiple linear regression was used to analyze the factors affecting HOMA-IR.Results Compared with the control group,the expression levels of serum FPG,FINS,HOMA-IR,RAGE and NF-κB in the GDM group were significantly increased(P<0.05);compared with the non-IR group,the expression levels of serum RAGE and NF-κB in the IR group were significantly higher(P<0.05).Pearson method showed that the expressions of RAGE and NF-κB in serum of GDM patients were positively correlated(r=0.664,P<0.05);the expressions of RAGE and NF-κB in serum of patients with GDM were positively correlated with HOMA-IR,FPG and FINS(P<0.05).The area under the curve of serum RAGE combined with NF-κB in predicting IR in patients with GDM
