摘　　要：目的基于Logistic回归分析重度子痫前期(PE)患者发生自发性流产的影响因素。方法回顾性收集广元市中心医院2018年8月至2020年8月期间收治的128例重度PE患者的临床资料作为研究对象,将其中发生自发性流产的患者纳入发生组(n=20),未发生自发性流产的患者纳入未发生组(n=108)。统计并比较两组的临床资料,分析重度PE患者发生自发性流产的影响因素。结果发生组有流产史患者占比多于未发生组,血清D-二聚体(D-D)、同型半胱氨酸(Hcy)水平高于未发生组,白蛋白(ALB)水平低于未发生组(P<0.05)。经Logistic回归分析显示,有流产史、血清D-D和Hcy过表达是重度PE患者发生自发性流产的危险因素(OR>1,P<0.05);血清ALB过表达是重度PE患者发生自发性流产的保护因素(OR<1,P<0.05)。结论有流产史、血清D-D和Hcy过表达是重度PE患者发生自发性流产的危险因素,血清ALB过表达是重度PE患者发生自发性流产的保护因素。Objective To analyze the influencing factors of spontaneous abortion in patients with severe preeclampsia(PE)based on Logistic regression.Methods A retrospective analysis was conducted,the clinical data of 128 patients with severe PE treated in Guangyuan Central Hospital from August 2018 to August 2020 were collected.Patients with spontaneous abortion were included in the occurrence group(n=20),and those without spontaneous abortion were included in the non-occurrence group(n=108).The clinical data of the two groups were statistically compared,and the influencing factors of spontaneous abortion in patients with severe PE were analyzed.Results The proportion of patients with history of abortion in the occurrence group was higher than that in the non-occurrence group,the serum D-dimer(D-D)and homocysteine(Hcy)levels were higher than those in the non-occurrence group,and the serum albumin(ALB)level was lower than that in the non-occurrence group(P<0.05).Logistic regression analysis showed that history of abortion and overexpression of serum D-D and Hcy were risk factors for spontaneous abortion in patients with severe PE(OR>1,P<0.05).Serum ALB overexpression was a protective factor for spontaneous abortion in patients with severe PE(OR<1,P<0.05).Conclusions History of abortion and overexpression of serum D-D and Hcy are risk factors for spontaneous abortion in patients with severe PE,while overexpression of serum ALB is a protective factor for spontaneous abortion in patients with severe PE.
