摘 要:目的:探讨妊高征患者血清高分子量脂联素(HMW-ADP)水平对滋养细胞p53稳态的调节和继发妊娠子痫发生的影响。方法:选择100例妊娠期高血压患者平均分为轻度、中度和重度先兆子痫患者三组,以32例健康孕妇作为对照组。测定各组孕妇血清HMW-ADP及血压水平、滋养细胞p53 mRNA和蛋白表达水平、增殖和迁移能力、继发妊娠子痫发生率和胎儿发育相关指标,同时分析血清HMW-ADP水平与滋养细胞p53稳态的调节和继发妊娠子痫发生的相关性。结果:各组孕妇血清HMW-ADP及血压水平、滋养细胞p53 mRNA和蛋白表达水平、增殖和迁移能力、继发妊娠子痫发生率和胎儿发育相关指标比较差异具有统计学意义(P<0.05)。随着孕妇患病程度加重,HMW-ADP和瘦素(LEP)水平越来越低,血清胱抑素C(CysC)和血清肌酐(Scr)水平、继发妊娠子痫发生率越来越高;随着时间延长,p53 mRNA和蛋白表达水平增加。经过相关性分析得知,血清HMW-ADP水平与滋养细胞p53稳态的调节和继发妊娠子痫发生之间存在显著的相关性。结论:妊高征患者血清HMW-ADP水平明显降低,且参与调节滋养细胞p53稳态化,提示低HMW-ADP水平可能参与了继发妊娠子痫的发生发展。Objective To investigate the effect of serum HMW adiponectin level on the regulation of trophoblast p53 homeostasis and the occurrence of eclampsia in pregnancy.Method 100 patients with hypertension during pregnancy were divided into three groups: mild, moderate and severe preeclampsia, and 32 healthy pregnantwomen were selected as the control group. Serum hmw adiponectin and blood pressure levels, trophoblast p53 mRNA and protein expression levels, proliferation and migration ability, incidence of secondary pregnancy eclampsia and fetal development were measured in each group. Meanwhile, the correlation between serum hmw adiponectin level and trophoblast p53 homeostasis and occurrence of secondary pregnancy eclampsia was analyzed. Results There were significant differences in serum HMW-ADP. and blood pressure levels, trophoblast p53 mRNA and protein expression levels, proliferation and migration ability, incidence of secondary pregnancy eclampsia and fetal development(P<0.05). With the aggravation of the disease in pregnant women, the levels of HMW-ADP and LEP were lower and lower, the levels of CysC and Scr and the incidence of secondary eclampsia were higher and higher. The expression levels of p53 mRNA and protein increased with time.The correlation analysis showed that serum HMW-ADP level was significantly correlated with the regulation of trophoblast cell p53 homeostasis and the occurrence of eclampsia in secondary pregnancy.Conclusion The serum HMW-ADP level was significantly reduced in patients with PIH, and was involved in the regulation of trophoblast cell p53 homeostasis, suggesting that low HMW-ADP level may be involved in the occurrence and development of secondary pregnancy eclampsia.
