摘 要:目的探讨氨基葡萄糖联合关节腔注射自体富血小板血浆(platelet-rich plasma,PRP)对膝关节骨性关节炎(knee osteoarthritis,KOA)患者Lysholm评分、关节液基质金属蛋白酶-3(matrix metalloproteinase 3,MMP-3)、关节液金属蛋白酶抑制剂-1(tissue inhibitors of metalloproteinase 1,TIMP-1)水平及数字评定量表(numeric rating scale,NRS)评分的影响。方法选取94例KOA患者,依据随机数字表法分为观察组(氨基葡萄糖联合自体PRP关节腔注射治疗)与对照组(单纯服用氨基葡萄糖治疗),各47例。比较2组临床疗效、Lysholm评分、MMP-3水平、TIMP-1水平及NRS评分。结果观察组总有效率93.62%,高于对照组的76.60%,差异有统计学意义(P<0.05)。治疗前,2组患者Lysholm评分差异无统计学意义(P>0.05);治疗后,2组Lysholm评分均较治疗前升高,且观察组高于对照组[(80.25±10.68)分vs(66.42±9.31)分],差异有统计学意义(P<0.05)。治疗前,2组患者关节液MMP-3、TIMP-1水平差异无统计学意义(P>0.05);治疗后,2组关节液MMP-3、TIMP-1水平均较治疗前改善,且观察组改善情况均优于对照组,差异有统计学意义(P<0.05)。治疗前,2组患者NRS评分差异无统计学意义(P>0.05);治疗后,2组患者NRS评分均较治疗前下降,且观察组低于对照组[(1.91±0.74)分vs(3.26±1.37)分],差异有统计学意义(P<0.05)。结论对KOA患者实施氨基葡萄糖联合自体PRP关节腔注射治疗,其临床疗效优于单用氨基葡萄糖治疗,可改善患者膝关节功能,减轻关节疼痛,对关节液MMP-3、TIMP-1水平起到有效调节作用。Objective To investigate the effect of glucosamine combined with intra-articular injection of autologous platelet-rich plasma(PRP)on Lysholm score,synovial fluid matrix metalloproteinase 3(MMP-3),the level of tissue inhibitors of metalloproteinase 1(TIMP-1)in synovial fluid and the score of numerical rating scale(NRS)in patients with knee osteoarthritis(KOA).Methods A total of 94 patients with KOA were selected and randomly divided into an observation group(treated with glucosamine combined with autologous PRP intra-articular injection)and a control group(treated with glucosamine alone),47 cases in each group.The clinical efficacy,Lysholm score,MMP-3 level,TIMP-1 level and NRS score were compared between the two groups.Results The total effective rate in the observation group was 93.62%,which was higher than 76.60%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in Lysholm scores between the two groups(P>0.05);after treatment,the Lysholm scores of the two groups were both higher than those before treatment,and the observation group was higher than the control group[(80.25±10.68)points vs(66.42±9.31)points],the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of MMP-3 and TIMP-1 in the synovial fluid between the two groups(P>0.05);after treatment,the levels of MMP-3 and TIMP-1 in the synovial fluid in the two groups were improved compared with those before treatment,especially in the observation group,and the difference between the two groups was statistically significant(P<0.05).Before treatment,there was no significant difference in NRS scores between the two groups(P>0.05);after treatment,the NRS scores in both groups decreased compared with those before treatment,and the observation group decreased more significantly than the control group[(1.91±0.74)points vs(3.26±1.37)points],and the difference was statistically significant(P<0.05).Conclusion The clinical ef
