摘　　要：目的探讨负压封闭引流(VSD)技术应用于老年患者难愈性创面中的临床效果。方法收集2017年2月至2020年1月遂宁市中心医院收治的110例难愈性创面老年患者的临床资料,根据治疗方式的不同将患者分为对照组(n=46,给予常规治疗)和观察组(n=64,在常规治疗基础上给予VSD治疗)。观察两组患者治疗后的临床疗效。比较两组患者治疗后的创面愈合时间和住院时间。采用视觉模拟量表(VAS)评估并比较治疗前后两组患者的疼痛程度。比较两组患者治疗前后的血清细胞间黏附分子-1(ICAM-1)和可溶性血管细胞黏附分子-1(sVCAM-1)水平。结果治疗后,观察组患者的总有效率高于对照组患者,临床疗效优于对照组患者,差异有统计学意义(P<0.05)。治疗后,观察组患者的创面愈合时间、住院时间均明显短于对照组患者,差异均有统计学意义(P<0.01)。治疗后1周,两组患者的VAS评分均低于本组治疗前,且观察组患者的VAS评分低于对照组患者,差异均有统计学意义(P<0.05)。治疗后1周,观察组患者的血清ICAM-1水平低于对照组患者,差异有统计学意义(P<0.05),但两组患者的血清sVCAM-1水平比较,差异无统计学意义(P>0.05)。结论VSD技术对老年患者难愈性创面具有较好的治疗效果,能够减轻患者疼痛程度,缩短创面愈合时间、住院时间,改善炎症反应,值得临床使用。Objective To investigate the clinical effect of vacuum sealing drainage(VSD)in elderly patients with refractory wound.Method The clinical data of 110 elderly patients with refractory wounds who were admitted to Suining Central Hospital from February 2017 to January 2020 were collected,and the patients were divided into a control group(n=46,given conventional treatment)and an observation group(n=64,given VSD treatment based on the conventional regimen).The clinical efficacy of the two groups of patients after the treatment was observed.The wound healing time and hospital stay after treatment were compared between the two groups.The visual analog scale(VAS)was used to evaluate and compare the pain degree of the two groups before and after the treatment.The levels of serum intercelluar adhesion molecule-1(ICAM-1)and soluble vascular cell adhesion molecule-1(sVCAM-1)were also compared between the two groups before and after the treatment.Result After treatment,the complete response rate of the observation group was higher than that of the control group,and the clinical efficacy was better than that of the control group,the difference was statistically significant(P<0.05).After the treatment,the wound healing time and hospitalization time of the observation group were significantly shorter than those of the control group,and the differences were statistically significant(P<0.01).One week after the treatment,the VAS scores of the two groups were lower than those before the treatment,and the VAS scores of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).One week after the treatment,the serum ICAM-1 level in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).However,there was no significant difference in the serum sVCAM-1 level between the two groups(P>0.05).Conclusion VSD has a good therapeutic effect on the refractory wounds of elderly patients.This technique,which is worthy of
