摘　　要：目的探讨血浆蛋白C(PC)和蛋白S(PS)在静脉血栓栓塞症(VTE)患者中的诊断价值。方法收集2018年11月至2020年5月三亚市中医院收治的82例VTE患者的临床资料,根据疾病类型分为深静脉血栓形成(DVT)组(n=59)和肺血栓栓塞症(PTE)组(n=23),选取同期住院的非VTE患者30例为对照组。分析PC、PS活性与DVT、PTE的关系,评估两指标单独及联合检测对DVT、PTE的诊断价值。结果3组患者PC、PS活性比较,差异均有统计学意义(P<0.05)。DVT组、PTE组患者PC、PS活性均明显低于对照组,差异均有统计学意义(P<0.01)。Spearman相关性分析显示,PC、PS活性与DVT、PTE的发生均呈负相关(P<0.05)。受试者工作特征(ROC)曲线分析结果显示,PC、PS两者联合诊断DVT、PTE的曲线下面积(AUC)均高于各单项指标。结论PC、PS活性在VTE患者机体血浆中呈下降趋势,其活性与VTE的发生率均呈负相关,两者联合检测对DVT、PTE均具有较高的诊断价值。Objective To investigate the diagnostic value of plasma protein C(PC)and protein S(PS)in patients with venous thromboembolism(VTE).Method The clinical data of 82 patients with VTE admitted to Sanya Traditional Chinese Medicine Hospital from November 2018 to May 2020 were collected and divided into deep vein thrombosis(DVT)group(n=59)and pulmonary thromboembolism(PTE)group(n=23)according to the type of disease.A total of 30 non-VTE patients hospitalized during the same period were selected as the control group.The relationship between PC and PS activities with DVT and PTE in the three groups were analyzed,the diagnostic value of the two indicators alone and combined for DVT and PTE were evaluated.Result The PC and PS activities of the three groups were compared,the differences were statistically significant(P<0.05).The activities of PC and PS in the DVT group and PTE group were significantly lower than those in the control group(P<0.01).Spearman correlation analysis showed that the activities of PC and PS were negatively correlated with the occurrence of DVT and PTE(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the area under the curve(AUC)of PC and PS for the combined diagnosis of DVT and PTE were higher than that of either indicator alone.Conclusion The activities of PC and PS tend to decrease in patients with VTE,and their activities are negatively correlated with the occurrence of DVT.The combined detection of the two indicators has a high value in the diagnosis of DVT and PTE.
