摘　　要：目的研究连花清瘟胶囊治疗社区获得性肺炎(community acquired pneumonia,CAP)患者的真实世界临床用药特征,为临床合理用药和优化治疗方案提供参考借鉴依据。方法基于中国中医科学院中医临床基础医学研究所建立的HIS大型数据库,提取2004年9月—2015年12月全国18家三甲医院应用连花清瘟胶囊治疗的1395例CAP患者的医疗数据,了解患者的人群特征、临床特征、诊断及病情转归情况,分析连花清瘟胶囊的用量、疗程以及联合用药特征,运用数据挖掘中关联规则Apriori计算方法研究联合用药的使用规则,探索真实世界的用药规律。结果对1395例使用连花清瘟治疗CAP患者进行分析研究,年龄在45~65岁的数量最多,集中在内科(90.39%),药物的使用符合说明书中推荐的给药途径及用药剂量范围,用药疗程大多是15~28 d。临床上常用的联合用药类别分别为,西药类别:液体支持、抗菌药、解热镇痛抗炎药、咳嗽和感冒药、解痉平喘药;中药类别:清热剂、祛瘀剂、化痰止咳平喘剂、扶正剂、解表剂。结论真实世界应用连花清瘟治疗CAP患者的用药特征符合CAP治疗指南,应用关联规则之Apriori算法探索其联合用药的治疗规律,为临床合理用药和进一步优化治疗方案提供借鉴依据,为今后的前瞻研究拓展了线索与思路。Objective To study the clinical effect and medication characteristics of Lianhua Qingwen Capsule(连花清瘟胶囊)in the treatment of community acquired pneumonia(CAP)in the real world so as to provide reference for clinical rational drug use and treatment optimization.Method Based on the database of HIS established by Institute of Clinical Basic Medicine of Chinese Academy of Chinese Medical Sciences,the medical data of 1395 CAP patients treated with Lianhua Qingwen Capsule from September 2004 to December 2015 in 18 class A hospitals in China were extracted to understand the population characteristics,clinical characteristics,diagnosis and prognosis of patients and analyze the dosage,course of treatment and combination therapy of Lianhua Qingwen Capsule.According to the characteristics of drug use,the Apriori calculation method of association rules in data mining is used to study the use rules of combined drug use and explore the real world drug use rules.Results A total of 1395 cases of CAP patients treated with Lianhua Qingwen Capsule were analyzed and studied.The number of patients aged 45~65 years old was the most,mainly in internal medicine department(90.39%).The use of drugs met the recommended route of administration and dosage range in the manual.The course of treatment was mostly 15~28 days.The commonly-used types of combination drugs in clinic were western medicine(liquid support,antibiotics,antipyretic,analgesic and anti-inflammatory drugs,cough and cold drugs,antispasmodic and antiasthmatic drugs)and traditional Chinese medicine(heat-clearing agent,removing blood stasis agent,phlegm-resolving,cough-relieving and asthma-relieving agent,righting agent and surface relieving agent).Conclusion The medication characteristics of Lianhua Qingwen Capsule in the treatment of CAP patients in the real world are in line with the treatment guidelines of CAP.The Apriori algorithm of association rules is applied to explore the treatment rules of its combination,which provides reference for clinical rational drug u
